Objectives:We have previously reported that the histological mode of invasion of oral squamous cell carcinoma (OSCC) is a significant risk factor for pain. We sought to determine whether pain is a risk factor for poor prognosis in patients with OSCC. Study Design:We evaluated the relationships between overall survival rates (OSRs) and clinicopathological variables, including gender, age, Tand N-stages, pathological findings and pain in 109 consecutive patients with untreated OSCC. Results:Of these 109 patients, 40 (37%) reported spontaneous pain.Univariate analysis showed that the OSR of patients with spontaneous pain was significantly lower than that of patients without pain (p=0.002).Multivariate analysis revealed that spontaneous pain and N-stage were significant independent predictors of OSR. Conclusion:This is the first report showing that spontaneous pain before treatment may be associated with poor prognosis in patients with OSCC.3

Objectives Pancreatic cancer is a lethal disease that offers little chance of long-term survival for patients with unresectable tumors. Surgery remains the most effective means of attaining prolonged survival, yet its role remains limited. Regional chemotherapy has been described for patients with pancreatic cancer, including reports of objective tumor regression allowing for tumor resection in previously unresectable cases, however comprehensive data have not been reviewed to date. Methods A review of the literature from 1995 to 2010 was performed to analyze the results of regional chemotherapy administered to patients with advanced pancreatic cancer. Reports of individual cases, post-operative regional therapy and treatment of mixed tumor types were excluded. Results Twenty-one reports of 895 total patients with pancreatic cancer were reviewed. Greater than 95% of patients had stage III or IV adenocarcinoma. Objective response rates ranged from nil to 58%, with associated median survivals of 4 to 22 months. Low grade gastrointestinal and hematologic toxicities were not uncommon. Conclusions Regional chemotherapy can be administered safely to patients with pancreatic cancer, but with unclear benefit. Advanced pancreatic tumors converted to resectable status by the use of regional chemotherapy may improve patient survival.

StreszczenieLeczenie raka trzustki jest poważnym problemem współczes -nej medycyny. Rak ten stanowi ok. 3% nowotworów wykrywanych corocznie na świecie i zajmuje 4. miejsce pod względem śmiertelności wśród nowotworów złośliwych. Agresywny przebieg, późne rozpoznanie oraz oporność na stosowane leczenie powoduje, że 5-letnie przeżycie nie przekracza 5%. Podstawową metodą terapii wczesnych postaci nowotworu jest radykalny zabieg, jednak w chwili rozpoznania u 80-90% chorych proces nowotworowy jest zaawansowany, a guz nieoperacyjny. Jedną z możliwości poprawy wyników leczenia raka trzustki jest stosowanie terapii uzupełniającej. W niniejszym artykule przedstawiono przegląd aktualnych badań z randomizacją dotyczących leczenia adiuwantowego, neoadiuwantowego, monoterapii, terapii skojarzonej oraz leczenia bólu czy roli odżywiania w terapii raka trzustki. Pancreatic cancer remains a highly lethal disease and is the 4 th leading cause of cancer-related deaths in the United States, with a 5-year survival of less than 5%. The poor prognosis of this cancer is due to its propensity for early local invasion and metastasis. Most patients present with advanced disease, precluding curative surgical resection, which remains the only potential curative approach. The adjuvant treatment may improve the prognosis in pancreatic cancer. This article presents a review of randomized clinical trials including adjuvant and neoadjuvant treatment, monotherapy and combination therapy, pain relief and the role of nutrition in pancreatic cancer treatment.
