ÖZETAmaç: Bu çalışmada kliniğimizdeki evre 1 over granüloza hücreli tümör (GHT) hastaların klinik verileri eşliğinde tedavi ve takip uygulamaları ile bunların sonuçları tartışılmıştır. Yöntem: Temmuz 2006 -Aralık 2015 yılları arasında kliniğimizde evre 1 GHT tanısı alan 15 hasta klinikopatolojik özellikleri, uygulanan tedaviler ve uzun süreli takip sonuçları açısından değerlendirilmiştir. Bulgular: Medyan yaşı 52 (24-71) idi. FIGO Evreleme Sistemine göre 9 hasta evre IA iken 6 hasta evre IC idi. Hastaların yarısı (53.3%) uterin kanama ile başvurdu. En sık (73.3%) yerleşim yeri ise sol over idi. Cerrahi tedavi olarak 10 hastaya total abdominal histerektomi+ bilateral salpingooferektomi+bilateral pelvik-paraaortik lenf nodu diseksiyonu+omentektomi, 2 hastaya sadece total abdominal histerektomi+bilateral salpingooferektomi ve 3 hastaya ise fertilite koruyucu cerrahi uygulanmıştı. 3 hastada perinöral invazyon (PNI) ve 2 hastada lenfovasküler invazyon (LVI) saptandı. Endometrium histopatolojik değerlendirilmesinde, 6 hasta ile en fazla basit atipisiz hiperplazi saptandı. Toplam yedi hastaya adjuvan tedavi olarak 3 kür [Bleomisin, Etoposid, Sisplatin (BEP)] kemoterapi rejimi verildi. Medyan takip süresi 77.9 ay (4.1-108.5 ay) ve genel sağkalım 77.9 ay (95% CI, 39.6-116.3) idi. Takiplerde hiçbir hastada nüks gelişmedi. Sonuç: Over GHT nadir görülmektedir. Erken evre hastalarda adjuvan tedavi uygulanacak hastaları belirlemek üzere yüksek riskli grubunu saptamak için randomize çok merkezli prospektif çalışmalara ihtiyaç vardır. Methods: Between July 2006 -December 2015, the clinical and pathological parameters, treatment features and long term follow up outcomes of 15 patients with stage 1 GCOC were evaluated retrospectively. Results: Median age was 52 (24-71).According to FIGO staging system, 9 patients had stage 1A disease and 6 patientshad stage IC GCOC. Half of (53.3%) of the patients presented with uterine bleeding (53.3%) and left over (73.3%) was the most common localization. Ten patients had abdominal hysterectomy-bilateral salphingoopherectomy-pelvic paraaortic lymph node dissection-omentectomy and whereas 3 patients had total abdominal hysterectomy-bilateral salphingoopherectomy and 2 patients had fertility sparing surgery as a surgical procedure. 3 patients had perineural invasion, 2 patients had lymphovascular invasion.Histopathological evaluation of endometrium revealed simple hyperplasia without atypia in 6 patients. As adjuvant setting, 7 patients received 3 cycles of chemotherapy [bleomycin +etoposid+ cisplatin (BEP)]. Recently characterized molecular events in uncommon gynaecological neoplasms and their clinical importance

The introduction of new sequencing technologies has resulted in the discovery of commonly mutated genes in uncommon cancers, including non-epithelial ovarian neoplasms and other rare gynaecological tumours, such as cervical embryonal rhabdomyosarcoma. In some of these neoplasms, mutations in certain genes are both frequent and specific enough for the genomic mutations and sometimes their associated protein loss or overexpression to be used as an aid to diagnosis. In this review, we contrast previous gene identification methods with newer ones, and discuss how the new sequencing technologies (collectively referred to as 'next-generation sequencing') have permitted the identification of specific molecular events that characterize several rare gynaecological neoplasms. We highlight the value of using sequencing to complement traditional pathological methods when diagnosing certain tumours, and provide practical advice to pathologists dealing with these neoplasms. We focus on adult granulosa cell tumours (somatic monoallelic mutations in FOXL2), Sertoli-Leydig cell tumours, gynaecological embryonal rhabdomyosarcomas (germline and somatic mutations in DICER1), and small-cell carcinoma of the ovary, hypercalcaemic type (biallelic mutations in SMARCA4). The new genetic findings provided by next-generation sequencing in these uncommon neoplasms have brought these disorders back into focus, and point the way towards new diagnostic, preventive and therapeutic avenues.
