A growing body of evidence has indicated that behaviors of cancers are defined by not only intrinsic activities of tumor cells but also tumor-infiltrating immune cells (TIICs) in the tumor microenvironment. However, it still lacks a well-structured and comprehensive analysis of TIICs and its therapeutic value in esophageal cancer (EC). The proportions of 22 TIICs were evaluated between 150 normal tissues and 141 tumor tissues of EC by the CIBERSORT algorithm. Besides, correlation analyses between proportions of TIICs and clinicopathological characters, including age, gender, histologic grade, tumor location, histologic type, LRP1B mutation, TP53 mutation, tumor stage, lymph node stage, and TNM stage, were conducted. We constructed a risk score model to improve prognostic capacity with 5 TIICs by least absolute shrinkage and selection operator (lasso) regression analysis. The risk score=−1.86∗plasma+2.56∗T cell follicular helper−1.37∗monocytes−3.64∗activated dendritic cells−2.24∗resting mast cells (immune cells in the risk model mean the proportions of immune cell infiltration in EC). Patients in the high-risk group had significantly worse overall survival than these in the low-risk group (HR: 2.146, 95% CI: 1.243-3.705, p=0.0061). Finally, we identified Semustine and Sirolimus as two candidate compounds for the treatment of EC based on CMap analysis. In conclusion, the proportions of TIICs may be important to the progression, prognosis, and treatment of EC.

PURPOSE: To explore the postoperative prognosis of patients diagnosed with esophageal squamous cell carcinoma (ESCC) of stage T1-3N0M0 using a survival prognostic model (SPM). PATIENTS AND METHODS: Patients diagnosed with stage T1-3N0M0 ESCC from two cancer centers—Sun Yat-sen University Cancer Center (SYSUCC-A/ training cohort: N = 555), SYSUCC-B/ internal validation cohort: N = 241) and Henan Cancer Hospital (HNCH/ external validation cohort: N = 170)—that had undergone esophagectomy between 1995 and 2015 were enrolled in this study. The primary clinical endpoint was overall survival (OS). We have identified and integrated significant prognostic factors for OS by univariate and multivariate Cox regression methods applied in the training cohort to build a SPM that could be validated in the validation cohorts.RESULTS: The OS evaluation by the SPM was comparable in three cohorts (SYSUCC-A: C-index 0.654, SYSUCC-B: C-index 0.630, HNCH: C-index 0.688). Discretization of patients was done using a fixed survival score cutoff of 136.434 based on our SPM determined from the training cohort divided into low- and high-risk subgroups with stratified OS in the validation cohort (SYSUCC-A: hazard ratio [HR] 1.009, 95% confidence intervals [CI], 1.006–1.011, P < 0.001; SYSUCC-B: HR 1.009, 95% CI, 1.004–1.013, P ˂ 0.001; HNCH: HR 1.010, 95% CI 1.005-1.015, P = 0.0017). The 48-month OS in the low-risk subgroup vs. that in the high-risk subgroup was 80.8% vs. 60.9% for SYSUCC-A, 86.4% vs. 60.7% for SYSUCC-B, and 89.7% vs. 64.1% for HNCH. CONCLUSION: We have established and validated a novel SPM that can predict the OS for T1-3N0M0 ESCC patients and could help clinicians to detect subgroups of patients with poor prognosis.

Background: No existing methods can accurately predict postoperative prognosis in patients with stage T1-3N0M0 esophageal squamous cell carcinoma (ESCC). We explored the postoperative prognosis of such patients using a new prognostic risk classification system (Esorisk). Methods: Patients with stage T1-3N0M0 ESCC from two cancer centers—Sun Yat-sen University Cancer Center (training cohort: N=819) and Henan Cancer Hospital (external validation cohort: N=177)—who underwent esophagectomy between 1995 and 2015 were included. We integrated significant risk factors for death events into multivariable logistic regression models and applied them to the training cohort to build Esorisk. The parsimonious aggregate Esorisk score was calculated for each patient; the training set was divided into three prognostic risk classes according to the 33rd and 66th Esorisk score percentiles. Esorisk’s association with cancer-specific survival (CSS) was assessed using Cox proportional hazard regression analyses. Results: The Esorisk model was: [10 + 0.023 × age + 0.517 × drinking history – 0.012 × hemoglobin – 0.042 × albumin – 0.032 × lymph nodes removed]. Patients were grouped into three classes—Class A (5.14-7.26, low risk), Class B (7.27-7.70, middle risk), and Class C (7.71-9.29, high risk). In the training group, the 5-year CSS decreased across categories (A: 63%, B: 52%, C: 30%; log-rank P<0.001). Similar findings were observed in the validation group (A: 81%, B: 69%, C: 48%; log-rank P<0.001). Esorisk stratified the CSS in patients from two cohorts with stage pT1 (P=0.0029), pT2 (P<0.001), or pT3 (P<0.001) disease. Cox proportional hazard regression analyses showed that the Esorisk aggregate score remained significantly associated with CSS in the training cohort (P<0.001) and validation cohort (P=0.001) after adjusting for pT stage and tumor differentiation. Conclusion: We established and validated a novel prognostic risk classification that can predict the CSS for patients with T1-3N0M0 ESCC. This may help clinicians identify patient subgroups with poor prognosis.
