Résumé -Introduction : le kératokyste odontogéne (KKO) est une des tumeurs odontogènes les plus fréquentes. Plusieurs travaux se sont intéressés à l'étude de cette lésion depuis sa première description afin de pouvoir comprendre son comportement biologique. Matériel et méthode : il s'agit d'une étude rétrospective immunohistochimique des marqueurs PCNA, Ki67 et P53 chez 40 patients opérés pour un KKO. Résultats : ces marqueurs sont retrouvés dans les couches suprabasales de l'épithélium de la paroi kystique. Il n'y a pas de différence statistiquement significative dans l'expression de ces marqueurs entre les différentes formes cliniques (sporadique, récurrent et syndromique), et il n'existe pas de corrélation entre ces marqueurs et la survenue d'une récidive. Conclusion : PCNA, Ki67 et P53 ne sont pas des marqueurs pour l'histopronostic des KKO.Abstract -Immunohistochemical study of proliferative (Ki67, PCNA) and apoptotic (P53) factors of 40 cases of odontogenic keratocysts. Introduction: the odontogenic keratocyst (OKC) is one of the most prevalent odontogenic tumours. Since its initial description, a number of studies have focused on different aspects of this lesion, attempting to explain its distinctive biological behavior. Patients and methods: a retrospective immunohistochemical studies of the expression of PCNA, Ki67 and p53 protein in 40 patients treated for OKC. Results: in the OKCs, the positivity of these markers was expressed mostly in the suprabasal layers of cystic epithelium. There is no statistically significant difference of expression between the different clinical forms (sporadic, recurrent and syndromic OKC), neither a correlation between the expression of this markers and recurrence. Conclusion: PCNA, P53 and Ki67 can not be considered as prognostic factors of OKC.

Résumé -Le kératokyste odontogène, maintenant considéré par certains auteurs comme une tumeur qu'ils ont appelé tumeur odontogène kératokystique, est une lésion kystique qui est le plus souvent localisée au sein des maxillaires. Cependant, quelques rares cas se développent dans la gencive. Cette variante, dénommée kératokyste odontogène périphérique, est souvent confondue cliniquement avec d'autres kystes odontogènes, notamment le kyste gingival de l'adulte, le kyste parodontal latéral ou le kyste radiculaire latéral. La variante périphérique, comme son homologue osseux, peut avoir un potentiel agressif et récidivant et elle peut aussi faire partie du syndrome de la naevomatose basocellulaire. Dans cet article, on rappelle brièvement développement de l'organe dentaire et on effectue la synthèse des cas publiés afin d'essayer de mieux comprendre la pathogénie et le diagnostic différentiel du kératokyste odontogène périphérique.Abstract -Peripheral odontogenic keratocyst: embryology reminder and literature review. The odontogenic keratocyst, considered today by some authors as a tumour and therefore called keratocystic odontogenic tumour, is a cystic lesion most frequently located within the jaws. Nevertheless, some rare cases have occurred in the gingiva. This form has been designated as peripheral odontogenic keratocyst. Such lesions are often misdiagnosed as gingival cyst of the adult, or lateral periodontal cyst or lateral radicular cyst. Peripheral odontogenic keratocyst is clinically aggressive and has the potential to recur as the classical odontogenic keratocyst. Furthermore it can also be observed in the nevoid basal cell carcinoma syndrome. In this paper we have briefly summarised the tooth development and we report the previously published cases.

Abstract: The results suggested that the evaluation of Ki-67 expression in KCOT at the time of pathological diagnosis might be helpful for consideration of appropriate adjunctive surgical procedures to avoid a recurrence and may serve as a prognostic marker.
