Prognostic factors determining survival in patients with node positive differentiated thyroid cancer: a retrospective cross‐sectional study

Abstract: Cervical node metastasis in thyroid carcinoma slightly reduces overall survival. Prognostic factors can help identify high-risk patients and point towards an adequate therapeutic approach.

“…CLNM is common in DTC, with a frequency of 30-90% (17) and has been demonstrated to be closely associated with higher recurrence and poorer survival rates in patients with DTC (18). The majority of metastatic sites are located in central compartment lymph nodes, and complete central lymph node dissection has been identified to reduce the incidence of disease persistence, recurrence and mortality (19).…”

“…In addition, we found p53 expression to be associated with several characteristics well recognized as better prognosis factors such as tumor size, presence of a tumor capsule, presence of thyroiditis, absence of multifocal tumors, and absence of metastasis at the time of diagnosis [20, 21]. …”

“…Regional LM is a poor prognostic factor which aggravates the prognosis of patients with DTC. 2,3 Therefore, current guidelines recommend the removal of all clinically suspected LM regions. 18 However, preoperative imaging modality including CT, magnetic resonance imaging, ultrasonography and F-18 FDG PET/CT on cervical region showed low sensitivity (30%-40%) for detection of regional LM.…”

“…Cervical regional lymph node (LN) metastasis is common in differentiated thyroid cancer (DTC) including papillary and follicular thyroid cancer . The DTC with regional LN metastasis (LM) has been associated with poor survival and high recurrence rate than the node‐negative DTC . Therefore, regional LM is an important clinical prognostic factor in DTC treatment.…”
