Sexually transmitted diseases (STDs) in children are not uncommon in India, though systematic epidemiological studies to determine the exact prevalence are not available. STDs in children can be acquired via sexual route or, uncommonly, via non-sexual route such as accidental inoculation by a diseased individual. Neonatal infections are almost always acquired intrauterine or during delivery. Voluntary indulgence in sexual activity is also an important factor in acquisition of STDs in childhood. Sexual abuse and sex trafficking remain the important problems in India. Surveys indicate that nearly half of the children are sexually abused. Most at risk children are street-based, homeless or those living in or near brothels. Last two decades have shown an increase in the prevalence of STDs in children, though most of the data is from northern part of the country and from major hospitals. However, due to better availability of antenatal care to majority of women, cases of congenital syphilis have declined consistently over the past two-three decades. Other bacterial STDs are also on decline. On the other hand, viral STDs such as genital herpes and anogenital warts are increasing. This reflects trends of STDs in the adult population. Concomitant HIV infection is uncommon in children. Comprehensive sex education, stringent laws to prevent sex trafficking and child sexual abuse, and antenatal screening of all the women can reduce the prevalence of STDs in children.

Background Syphilis causes considerable morbidity not only in adults but also in children. Unfortunately, acquired syphilis in children has not received due attention and remains a relatively ignored field. Methods This was a retrospective study, in which epidemiologic, clinical, and investigational data were analyzed from medical records of all sexually transmitted infection (STI) cases attending the STI clinic of Pt. B. D. Sharma, PGIMS, Rohtak from January 1992 to November 2018. Children of age ≤14 years with acquired syphilis were studied. Results Of 9138 STD cases registered from January 1992 to November 2018, 39 were children with acquired syphilis, of whom 22 were males and 17 were females. The majority were in 12‐14 years age group (22 cases; 56.41%). A history of sexual abuse could be elicited in 10 patients. Three gave a history of voluntary sexual activity. Examination revealed secondary syphilis in 32, latent syphilis in 6 and only one case of primary chancre. Analysis of incidence of pediatric acquired syphilis cases over three‐year intervals during the last 27 years indicated an initial decreasing trend followed by recent slight rise in last 3 years. Conclusion Social inhibitions and reluctance to report child sexual abuse may have led to an underestimate of actual prevalence. The prevalence in the 12‐ to 14‐year age group may indicate emerging trends of promiscuous sexual behavior. The recent rising trend in occurrence of childhood acquired syphilis may mirror the recent resurgence of adult syphilis.

A cross-sectional observational study on child sexual abuse

Background: A child sexual abuse (CSA) incident never comes into limelight unless it is a rape, probably because it could be an asymptomatic event causing only an emotional trauma or due to fear of revealing. It has been observed that sexually transmitted infections (STI) are in increasing trend among adolescents due to early sexual maturity, increased promiscuity and sexual offenses.Methods: An observational cross-sectional study was carried out for a group of 774 school children between the age group of 14-17 years using a structured questionnaire to estimate the level of unreported child sexual abuse and sexual awareness. Descriptive statistical analysis was carried out in this study (SPSS 21.0).Results: It was observed that 5.5% of children had been abused among which 37.3 % and 62.7% were males and females respectively. 11.1% and 6.25% were abused in penetrative, 59.25% and 43.75% in non-penetrative forms among female and male victims respectively. Abusers were mainly family and friends (25.5%) followed by strangers (23.2%). 25.3% of unaffected and 27.9% of affected was aware of POSCO law. 88.3% of affected and 62.3% of unaffected rated good and above for the questionnaire.Conclusions: Unreported and silent victims have to be identified and addressed to prevent crimes and provide a safer environment for children. Implementation of POCSO law at the root level with sex education must be strengthened. Our study concluded that CSA is an extensive problem and even the lowest prevalence includes a huge number of victims who still need to be considered.
