Highlights: Percutaneous nephrolithotomy (PCNL) in older patients is as effective and safe as in younger patients. It is a viable option for managing renal stones in older patients with indications, although blood transfusions are frequently required. Abstract: Percutaneous nephrolithotomy (PCNL) for renal and proximal ureteral stone treatment among the elder population is considered challenging due to the complication risk associated with comorbidity and lower functional reserve. Patients older and younger than 60 years old were compared for efficacy and safety in following PCNL procedures. Consecutive patients who underwent PCNL from 2019-2021 in a single center were divided into patients aged at least 60 years (group I) and patients aged under 60 years (group II). Single stage fluoroscopic-guided PCNL were used for the entire study population. Patients’ habitus, stone-related, and operative characteristics were compared. The two groups’ PCNL success and complication rates were evaluated. A total of 245 patients, comprising 65 in group I and 180 in group II, were included for analysis. Diabetes mellitus prevalence was higher in group I (30.8% vs 18.9%). However, the study population did not show a significant difference in regard to comorbidity. Operative time, success rate (80% vs 74.4%), and complication rate (16.9% vs 15.6%) did not statistically differ (p>0.05). Transfusion rate was higher among patients aged at least 60 years (p=0.018). Based on the multivariate analysis, stones located in the renal pelvis was the factor which contributed to the success rate. This study’s results concluded that percutaneous nephrolithotomy is a safe and effective procedure for treating renal and proximal ureteral stones in the older population. Blood transfusions are more frequently given, in part, due to bleeding risk among older patients.

Magnesium and its alloys are promising candidates for degradable materials with good biocompatibility. Alloys based on Mg-Zn-Fe-Cu-Co compositions were designed using the equiatomic method of high entropy alloy. This paper discusses the microstructure and mechanical properties of these new high entropy alloys. Pure Magnesium (60 µm), Zinc (45 µm), Fe (10 µm), Cu (63 µm), and Co (1 µm) powder were mixed and milled using a shaker mill at 700 rpm for the 1800s. The resulting milled powders were compacted and sintered at 300 MPa for 180s and 600 MPa for 120s. Sintering was performed at 700oC for 2 hours in a tube vacuum furnace at a 5 °C/min heating rate under a high purity argon atmosphere. Microstructural analyses and mechanical tests were performed based on the American standard of testing and measurement. The alloys were basically multiphase and crystalline. The 20Mg-20Zn-20Fe-20Cu-20Co alloy consisted of the HCP phase and cubic phase. The physical and mechanical properties of Mg-Zn-Fe-Cu-Co were affected by the magnesium content in the matrix alloys. The presence of pores indicated uncomplete compaction and sintering process. The alloys have a medium hardness of between 286.06 HV - 80.98 HV, while the densities of the alloys were relatively moderate in the range of 3.057 g.cm-3 to 1.71 g.cm-3. Solid solution and intermetallic precipitation strengthening were believed the primary strengthening mechanics of the alloys. It is concluded that high entropy is a promising method for the processing of Mg alloys. Alloy with a chemical composition of 20Mg-20Zn-20Fe-20Cu-20Co had optimal mechanical properties that meet the minimum requirements of high entropy alloys as candidates for ureteral implant applications.

This study purpose to analyze the differences in the effectiveness of PCNL and open surgery in patients with staghorn stones. We searched the literatures from PubMed and ScienceDirect from year 2005 until 2020. The method used in this study was a systematic review with a quantitative statistical approach (meta-analysis) using primary research data. From 4 studies there were 148 cases of PCNL and 98 cases of open surgery. PCNL had lower postoperative stone-free rate than open surgery (OR 0.168). PCNL had a lower final stone-free rate than open surgery (OR 0.603). The number of patients who had complications with PCNL was lower than open surgery (OR 0.451). The number of patients receiving blood transfusions on PCNL was lower than for open surgery (OR 0.494). Patients who received PCNL procedure required a shorter hospital stay than open surgery (MD -3,234). The number of patients who received additional therapy modalities on PCNL was lower than open surgery (OR 1.917). The conclusions obtained in this study indicate that there are differences in the effectiveness of PCNL and open surgery for patients with staghorn stones.
