Profile of infective endocarditis observed from 2003 - 2010 in a single center in Italy

Abstract: BackgroundThis study aimed to provide a contemporary picture of the epidemiologic, clinical, microbiologic characteristics and in-hospital outcome of infective endocarditis (IE) observed in a single center in Italy.MethodsWe performed a retrospective study of patients with definite or probable IE observed at the “L. Sacco” Hospital in Milan, Italy, from January 1, 2003 through December 31, 2010.Results189 episodes of IE in 166 patients were included. The mean number of incident IE in the study period was of 1.… Show more

“…There are coding issues and different practices among doctors and hospitals; a better standardization is needed. Merging the two data-bases allowed a quality control and greater accuracy; this may explain why the incidence rate was higher than previously reported by other prospective European studies (13,23). Hoen estimated an incidence of 3,1/100.000 per year but an under-reporting may have been due to the absence of an "active search" of cases by investigators (13).…”

“…Streptococcus viridans and enterococcal IE were mainly communityacquired, whereas enterobacteriaceae/gram negative forms were nearly equally distributed between community and nosocomial origin. Incidence of staphylococcus aureus IE did not increase in the study period, P=0.95, not confirming previous studies results (10,22,23). Culture negative IE's were 11%, a lower prevalence than recently reported in European countries (24,25).…”

“…[1][2][3] IE also frequently causes debilitating morbidities, such as heart failure, stroke and renal failure requiring dialysis, which contribute to increased mortality and disability adjusted life years (DALYs). [3][4][5][6][7] The spectrum of causative microorganisms and underlying risk factors for IE has shifted dramatically in high income countries (HICs). 8,9 Staphylococcal IE now predominates.…”
