Background: Comprehensive Geriatric Assessment (CGA) is a comprehensive, multi-dimensional biopsychosocial examination. The CGA is conducted at the primary health facility such as the public health centers (Pusat Kesehatan Masyarakat, Puskesmas); categorized as Eldery-Friendly Public Health Centers (Puskesmas Santun Lansia), a puskesmas where one of its programs provides health care for the elderly, and Puskesmas in general, where there is no specific program for the elderly. The purpose of the study was to analyze the difference between Puskesmas santun lansia and Puskesmas in providing CGA. Methods: A comparative analytical study was carried out from August to November 2019, involving Puskesmas Santun Lansia (n=6) and puskesmas (n=12) in Bandung City. From each Puskesmas, two elderly were selected and observed for the CGA. The assessment consisted of the identity of the elderly, anamnesis, vital sign examination, physical examination, assessment of nutritional status, functional, psychosocial, social, cognitive, and mental assessment. The collected data were analyzed using the Chisquare or Fisher's exact test. Results: This study showed that there was no difference between Puskesmas Santun Lansia and Puskesmas in the CGA, except in the simple nutritional anamnesis (p=0.003), anamnesis of the symptoms or signs of the dysfunction of the organ system (p=0.034), and body mass index measurement (p=0.009). Conclusions: The CGA is not well implemented in both Puskesmas. In the future, both of the Puskesmas should enhance their knowledge and skills to provide the CGA as mentioned in the regulation of the Minister of Health No. 67/2015.

The older persons in our society are a special group of people in need of additional measures of care and protection. They have medical, financial, emotional and social needs. The novel Coronavirus disease 2019 (COVID-19) only exacerbates those needs. COVID-19 is a new disease, and there is limited information regarding the disease. Based on currently available information, older persons and people of any age who have serious underlying medical conditions may be at higher risk of severe illness from COVID-19. Family physicians provide care for individuals across their lifespan. Because geriatricians are internists or family physicians with post-residency training in geriatric medicine, they are major stakeholders in geriatric care. The authors are concerned about the absence of a COVID-19 response guideline/special advisory targeting the vulnerable population of older adults. The management and response to COVID-19 will be implemented in part based on the local context of available resources. Nigeria has been described as a resource-constrained nation. Infection prevention in older persons in Nigeria will far outweigh the possibilities of treatment given limited resources. The aim was to recommend actionable strategies to prevent COVID-19-related morbidity or mortality among older persons in Nigeria and to promote their overall well-being during and after the pandemic. These recommendations cut across the geriatric medicine domains of physical health, mental health, functioning ability and socio-environmental situation.

A scoping review of age-related health conditions among geriatric populations in sub-Saharan Africa

Background Globally people are living longer, resulting in an increased demand for primary care services to be responsive to geriatric health needs. There is however, little data on the nature and prevalence of age-related health conditions among people aged 60 years and older in the sub-Saharan African (SSA) region. This scoping review was undertaken to synthesise evidence on the scope and nature of age-related health needs among people aged 60 years and older in SSA, in order to inform reforms in health systems and health professions education. Methods We systematically searched five electronic databases for studies published prior to 30 June 2019. Using the keywords “Africa”, “older adult” or “geriatric”, and “health”, two independent reviewers used a set of eligibility criteria to extract and screen the abstracts and full-text publications. Particular focus was given to age-dependent disorders in people over 60 years such as dementia, disability and falls. Results Sixty-four studies met the inclusion criteria. The review revealed a noticeable absence of studies from the majority of Sub-Saharan African countries. Publications included for review represent twelve countries from this region. The morbidities reported on included dementia, functional impairment, mental health, falls, sarcopenia and nutritional disorders, sensory impairment, frailty and musculoskeletal disorders. Many of these conditions co-existed among older adults, resulting in impaired quality of life and disability. Modifiable factors for geriatric morbidity such as hypertension, depression, cognitive and sensory impairments were under-diagnosed and often untreated. Conclusions The body of empiric research and evidence on age-related health conditions in geriatric people in SSA remains small. The prevalence of reported age-related health conditions was comparable to that in high-income countries. However, chronic conditions in the elderly were underdiagnosed and undertreated, often resulting in disability. An urgent review of current health systems and health professions training is required, taking into consideration the findings of this study.
