Introduction: Obstetric fistula (OF) is an abnormal communication between the genital and urinary tracts and/or associated with a recto-vaginal communication resulting from prolonged dystocic delivery. It is a frequent pathology in underdeveloped countries. In Zinder no study has been carried out on obstetric fistula. The aim of this study was to evaluate the epidemiological, social impact and therapeutic aspects of obstetric fistula at the CSME of Zinder. Patient and Method: This was a descriptive cross-sectional study of obstetric fistula care at CSME during the period of (January 2018 to June 2020). Results: A total of 196 cases were collected over 30 months. This represents an annual incidence of 78.4 fistulas. The median age of the patients was 18.63 years, and more than 65% were over 20 years old. One hundred and ninety-one patients (97.45%) did not attend school, and 56.12% (n = 110) lived in polygamous households. One hundred and fifty-one women (n = 151), 77.04% had full assistance from their husband during the first four (4) months of the pathology. Twenty-four (12.24%) had been notified of repudiation. Obstetrical risk of dystocia was found in 39.79% (n = 78) of the patients, of whom 24.49% (n = 48) had a focused antenatal consultation (CPNR). The labour lasted more than 24 hours in 100%. The majority of women are multiparous and 60% had at least 2 pregnancies. vesicovaginal fistula is the main pathological type with 86.23%. Fistulas were closed in 83.16% and 68.87% exited without urine loss. Conclusion: Obstetric fistula is a devastating disease affecting girls and young women in Niger, as in other countries in Africa and Asia. It is a major concern worldwide and is a social tragedy because it is disabling and depressing.

Introduction: Obstetric Fistula is defined as direct communication between the vagina and the bladder (vesicovaginal fistula) and/or between the vagina and the rectum (recto-vaginal fistula). Each year 50.000 to 100.000 new cases of obstetric fistula are globally reported. According to World Health Organization, more than 200 million young women live with unrepair obstetric fistula mostly in sub-Saharan Africa and southeast Asia including Afghanistan. Women with obstetric fistula suffer from frequent urinary or fecal incontinence which causes shame, social segregation, and other health problems. Obstructed labor (due to early marriage and inadequately developed pelvic) constitutes the most common cause of obstetric fistula. Therefore, the current study strived to assess teenage pregnancy on obstetric fistula among women in the fistula center of Malalai Maternity Hospital in Kabul, Afghanistan. Methods: A descriptive cross-sectional study was conducted over a period of one year (2019-2020) with 30 cases of obstetric fistula among women attended at Fistula Center of Malalai Maternity Hospital. Result: Among 30 women current aged with obstetric fistula 66.6 % were between20-37years, the mean age was 33.9±10.2 and the median was 30 years, the max and min range was between 20-56y. 66.6 % cases their marriage-age was between 16-20 y, while marriage age(mean) was 18.4±3.2, most women (60%) their ages in first delivery were between 15-19 y old, the height of the 40% women was less than 150 cm, and 46.7% women had 1-4 parity with mean of 5.2±3.1 and extreme (1-14), duration of labor in 90% were more than one day, majority of cases (60%) was Recto vaginal, 33% vesicovaginal and 7% with a compound type of fistula, 80% of cases were from remote and rural areas. Conclusions The teenage pregnancy and early marriage between (16-20 y old) was the vast majority of obstetric fistula formation, rectovaginal fistula, living in a remote area, short stature less than 150cm, and duration of more than one day have been seen. teenage pregnancy and obstetric fistula are preventable conditions. These findings suggest that efforts to reduce obstetrical fistula should target teenagers

Summary Background Rectovaginal fistulas represent 5% of all anorectal fistulas. For affected women, this pathology is associated with a reduction in quality of life (QoL) and self-esteem. Most commonly used methods of surgical closure have high recurrence rates or permanent perineal complaints, which in turn lead to negative effects on QoL and self-esteem. A fistula closure, using the “de-epithelialized Singapore flap” (SF), can be a good alternative therapy strategy. Method Our retrospective case series processes the long-term results of seven patients who were operated on for ano-/rectovaginal fistula using the SF. All patients underwent surgery at the University Hospital Graz, between May 2012 and July 2015. The data of the surgical follow-up examinations were collected and an additional telephone survey was carried out. The procedure is presented based on a structured description. All procedures were performed jointly by the Department of General Surgery and the Department of Plastic Surgery. Results The average age of the seven patients was 46.14 years (23–72 a). Five patients had a total of 12 previous operations with frustrating results. Of the seven patients treated, six had a permanent fistula closure (85.7%). The results of the telephone survey (n = 6) showed a high level of patient satisfaction (100%), and an improvement in QoL (83.3%), through our surgical method. In our cohort, neither urinary nor fecal incontinence occurred. Conclusion The treatment of an ano-/rectovaginal fistula using the “de-epithelialized pudendal thigh flap” (Singapore flap) is a promising treatment alternative. In particular, patients who have had previous proctological interventions show a benefit from this procedure.
