BackgroundAcute diffuse peritonitis is a common surgical emergency worldwide and a major contributor to non-trauma related death toll. Its causes vary widely and are correlated with mortality. Community acquired peritonitis seems to play a major role and is frequently related to hollow viscus perforation. Data on the outcome of peritonitis in the tropics are scarce. The aim of this study is to analyze the impact of tropic latitude causes of diffuse peritonitis on morbidity and mortality.MethodsWe retrospectively reviewed the records of 305 patients operated on for a diffuse peritonitis in two regional hospitals in the South-West Region of Cameroon over a 7 years period. The contributions of various causes of peritonitis to morbidity and mortality were analyzed.ResultsThe diagnosis of diffuse peritonitis was suggested on clinical ground only in more than 93 % of cases. The most common causes of diffuse peritonitis included peptic ulcer perforation (n = 69), complications of acute appendicitis (n = 53) and spontaneous perforations of the terminal ileum (n = 43). A total of 142 complications were recorded in 96 patients (31.5 % complication rate). The most common complications included wound dehiscence, sepsis, prolonged paralytic ileus and multi-organ failure. Patients with typhoid perforation of the terminal ileum carried a significantly higher risk of developing a complication (p = 0.002). The overall mortality rate was 15.1 %. The most common cause of death was septic shock. Differential analysis of mortality of various causes of peritonitis indicated that the highest contributors to death toll were typhoid perforation of terminal ileum (34.7 % of deaths), post-operative peritonitis (19.5 %) and peptic ulcer perforation (15.2 %).ConclusionThe diagnosis of diffuse peritonitis can still rely on clinical assessment alone in the absence of sophisticated imaging tools. Peptic ulcer and typhoid perforations are still major contributors to death toll. Patients presenting with these conditions require specific attention and prevention policies must be reinforced.

IntroductionLa péritonite communautaire est une urgence chirurgicale récurrente chez l'adulte qui constitue une préoccupation majeure pour le chirurgien et l'anesthésiste-réanimateur dans sa prise en charge. L'objectif de cette étude était d’établir non seulement le profil bactériologique des péritoniques communautaires opérées dans le service de chirurgie générale et digestive du CHU-YO mais aussi d’évaluer la sensibilité aux antibiotiques des souches bactériennes isolées à partir de ces dernières.MéthodesCent six (106) patients ont été recrutés dans cette étude et des prélèvements bactériologiques préopératoires ont été effectués dont 63 se sont révélés positifs.RésultatsSur les 63 prélèvements positifs, 78 germes ont été isolés soit une moyenne de 1,2 germe par échantillon. Escherichia coli été le germe le plus fréquemment isolé (33,3%), suivi des anaérobies (11,5%), Streptococcus sp (9%), Klebsiella pneumoniae (6,4%) et Staphylococcus sp (5,1%). Les antibiotiques les plus efficaces sur les bactéries identifiées dans les péritonites communautaires étaient respectivement l'imipenème (100%), la colistine (100%), la céftriaxone (100%), et la ciprofloxacine (65,4%)ConclusionLe profil de sensibilité des bactéries identifiées dans les principales péritonites communautaires aux antibiotiques montre une augmentation inquiétante du nombre de souches résistantes, notamment à l'association amoxicilline/acide clavulanique

Antimicrobial resistance among GLASS pathogens in Morocco: an epidemiological scoping review

Background Monitoring of antimicrobial resistance (AMR) is of great importance due to the frequency of strains becoming increasingly resistant to antibiotics. This review, using a public health focused approach, which aims to understand and describe the current status of AMR in Morocco in relation to WHO priority pathogens and treatment guidelines. Methods PubMed, ScienceDirect and Google Scholar Databases and grey literature are searched published articles on antimicrobial drug resistance data for GLASS priority pathogens isolated from Morocco between January 2011 and December 2021. Articles are screened using strict inclusion/exclusion criteria. AMR data is extracted with medians and IQR of resistance rates. Results Forty-nine articles are included in the final analysis. The most reported bacterium is Escherichia coli with median resistance rates of 90.9%, 64.0%, and 56.0%, for amoxicillin, amoxicillin-clavulanic acid, and co-trimoxazole, respectively. Colistin had the lowest median resistance with 0.1%. A median resistance of 63.0% is calculated for amoxicillin-clavulanic acid in Klebsiella pneumonia. Imipenem resistance with a median of 74.5% is reported for Acinetobacter baumannii. AMR data for Streptococcus pneumonie does not exceed 50.0% as a median. Conclusions Whilst resistance rates are high for most of GLASS pathogens, there are deficient data to draw vigorous conclusions about the current status AMR in Morocco. The recently join to the GLASS system surveillance will begin to address this data gap.
