A nonparametric Data Envelopment Analysis (DEA) is performed on hospitals in the federal state of Saxony (Germany) and in Switzerland. This study is of interest from three points of view. First, contrary to most existing work, patient days are not treated as an output but as an input. Second, the usual DEA assumption of a homogeneous sample is tested and rejected for a large part of the observations. The proposed solution is to restrict DEA to comparable observations in the two countries. Finally, hospital beds are treated as a discretionary input in one DEA and as a fixed input in the other, and the effect on efficiency is related to differences in hospital planning in Germany and Switzerland. Based on the comparable observations, hospitals of Saxony have higher efficiency scores than their Swiss counterparts. Terms of use: Documents inJEL-Classification: I11, I18, C61

Exploring the duality between a return to dollar definition of profit and the generalized distance function we establish the relationship between the Laspeyres, Paasche and Fisher productivity indexes and their alternative Malmquist indexes counterparts. By proceeding this way, we propose a consistent decomposition of these productivity indexes into two mutually exclusive components. A technical component represented by the Malmquist index and an economical component which can be identified with the contribution that allocative criteria make to productivity change. With regard to the Fisher index, we indicate how researchers can further decompose the Malmquist technical component rendering explicit the sources of productivity change. We also show how the proposed model can be implemented by means of Data Envelopment Analysis techniques, and illustrate the empirical process with an example data set.

This paper analyses productivity growth in health care delivery in Sweden and the impact of health care policy initiatives on productivity. In particular we consider the maximum waiting time guarantee introduced in Sweden 1992. The intention of the maximum waiting time guarantee was to shorten the waiting lists, partly through increased productivity. We measure productivity growth by estimating Malmquist productivity indices using non-parametric linear programming models. The productivity models are applied to data from a sample of public ophthalmology departments from 1988 to 1993. Positive productivity changes are found in several years, although no significant differences between the periods are found.
