Processes, contexts, and rationale for disinvestment: a protocol for a critical interpretive synthesis

Abstract: BackgroundPractical solutions are needed to support the appropriate use of available health system resources as countries are continually pressured to ‘do more with less’ in health care. Increasingly, health systems and organizations are exploring the reassessment of possibly obsolete, inefficient, or ineffective health system resources and potentially redirecting funds to those that are more effective and efficient. Such processes are often referred to as ‘disinvestment’. Our objective is to gain further unde… Show more

“…Unlike traditional systematic reviews that often focus on questions of effectiveness, CIS is helpful in generating midrange theories with strong explanatory power. (20,21) This is suitable for our goal of developing a conceptual framework that better integrates ndings from diverse elds and affords the opportunity to critically inspect both individual studies and the literature from each eld as a whole in terms of the nature of the assumptions underlying each eld, and what has in uenced their proposed solution. (22) The method begins with a compass question, which evolves throughout the course of the review.…”

“…The study was grounded in a combination of Social Determinants of Health (30), the '3-i theory' and the 'Policy Triangle Framework' (31). The '3-i' theory argues that effective policy setting, and implementation requires: 'idea'the appropriate knowledge; the right set-up and capable 'institute'; and 'interest' groups that promote a policy (32). The Policy Triangle Frameworks (PTF) recommends analysis of content, context and process in policy researching (33).…”

“…The first is broad and includes every element of healthcare delivery [ 22 , 61 , 85 , 86 ]. The second uses only a selection of these elements [ 12 , 42 , 87 90 ]. The third does not use a specific definition but suggests that health technologies are separate from other elements by including ‘health technologies’ within a list of selected items [ 27 , 45 , 49 , 51 , 53 , 91 93 ].…”

“…Disinvestment is frequently presented as if it is a new paradigm for health improvement. It has been described as an ‘emerging field’; disinvestment approaches, processes and initiatives are discussed; ‘research agendas’ are considered; and the need for mechanisms, frameworks, methods and tools are widely acknowledged [ 26 , 35 – 39 , 47 , 49 – 51 , 56 , 88 , 90 , 91 , 98 , 101 , 105 , 123 ]. However there are several health improvement paradigms with mature frameworks and validated methods and tools that are widely-used and well-accepted in local health services that already undertake disinvestment-type activities.…”
