Procalcitonin used as a marker of infection in the intensive care unit

Abstract: ProCT is not a better marker of infection than CRP in critically ill patients, but it can represent a useful adjunctive parameter to identify infection and is a useful marker of the severity of infection.

“…-Procalcitonin has been proposed as a marker of infection [60][61][62][63]. Furthermore, it may be useful as an indicator of the severity of infection and as a guide for therapy (dose de-escalation [64]) in respiratory infections [61,62,[65][66][67][68].…”

“…Performance of some biological markers of sepsis. 10,24,25 acute phase reactants have been described in sepsis, however none are specific enough for infection, as all can be increased in other inflammatory conditions without obvious infection (table 3).…”

“…[6][7][8] However, several investigators have questioned the diagnostic and prognostic accuracy of routine procalcitonin measurements, reporting inconsistent and variable results depending on the severity of illness and degree of infection in the patient population studied. [9][10][11] The triggering receptor expressed on myeloid cells-1 (TREM-1) is a recently discovered member of the immunoglobulin superfamily. 12 Like other similar cell-surface receptors, TREM-1 has a short intracellular domain and, when bound to its as yet unidentified ligand, associates with a signal-transduction molecule, DAP12, triggering the secretion of inflammatory cytokines that amplify the host response to microbial agents.…”

“…PCT was found to be an accurate diagnostic parameter for differentiating SIRS and sepsis [5]. Previously, PCT was reported to be a better discriminator than CRP in characterizing the degree of inflammation related to infection [17], especially infection complicated by sepsis or organ failure [18]. With this background, PCT has been proposed as a better prognostic marker than CRP [19,20].…”
