Procalcitonin kinetics in pediatric patients with systemic inflammatory response after open heart surgery

Abstract: In the SIRS+OF group peak procalcitonin levels were found to be highly predictive for mortality and organ failure development, whereas C-reactive protein levels were not. Daily procalcitonin measurements in SIRS+OF patients may help identify the postoperative infection during the follow-up period.

“…In paediatric patients, PCT levels increased more markedly after surgical repair of Tetralogy of Fallot than in those undergoing repair of ventricular septal defect or atrial septal defect [50]. Intraoperative factors have also been shown to influence the postoperative evolution of serum PCT levels (for example, aortic cross-clamping time [30,46,48,49], duration of CPB [30,45,46], and the duration of surgery [45]).…”

“…C-reactive protein (CRP) levels increase postoperatively, peaking between postoperative days one and three and remaining elevated up to the second week postoperatively [29,30,35,54]. irrespective of the extent of surgery [33,34,36,38,42,45,46,54].…”

“…24,62,63 Meisner et al 24 found that PCT was elevated above the reference range in 59% of patients after cardiothoracic surgery and in 95% of patients after gastrointestinal surgery. Although PCT serum levels rarely exceeded 2 ng/mL in uncomplicated cardiovascular surgery, levels in the range of 10 to 20 ng/mL were reported after major abdominal surgeries.…”

“…As with mechanical injury, very high initial levels, persistent high levels, or a second peak in serum PCT may predict the onset of sepsis. 62,63 Burns Serum PCT is frequently increased in patients with burn injuries as well. Carsin et al 64 measured the PCT levels in 40 burn patients with total body surface area burns greater than 30% and reported elevated levels within 4 hours after burn injury.…”

“…Procalcitonin levels may be useful to distinguish bacterial infections from nonbacterial infections (Assicot et al, 1993;Becker et al, 2008). They may help guide therapy and reduce antibiotic use, which can help save on cost of antibiotic prescriptions and drug resistance (Viallon et al, 2005;Celebi et al, 2006;Watkin et al, 2007).…”
