Procalcitonin and intestinal ischemia: A review of the literature

Abstract: Intestinal ischemia is common after emergency gastrointestinal or cardiovascular surgery. At present, there are no diagnostic tools for the early diagnosis of intestinal ischemia. In the last decade, procalcitonin (PCT) has been suggested as a marker of this condition. Here, we review the use of PCT as a diagnostic tool for intestinal ischemia. Two reviewers independently searched the PubMed and EMBASE databases for articles on intestinal ischemia and PCT. They then considered (1) the criteria applicable to pr… Show more

“…Procalcitonin may have some value in distinguishing bacterial infections from other sources of inflammation,8081828399 but the data are not sufficient to recommend the routine use of procalcitonin in the diagnosis of secondary peritonitis or in the identification of patients with bowel compromise. Similarly, C reactive protein may have a role in predicting the need for surgery in patients with abdominal pain,100101 or in identifying post-operative patients who develop a septic complication,102103104105 but the data are insufficient to recommend routine clinical use.…”

“…This precursor of the calcitonin is currently used in clinical practice for the differential diagnosis of infection of bacterial origin. According to a systematic review by Cosse et al [49], PCT's positive and negative predictive values for the diagnosis of acute i3 ranged between 27% and 90% and between 81% and 100%, respectively. As underlined by the authors, PCT is usually elevated during sepsis, in specific bacterial infections, and in various types of ischemias.…”

“…При бактериемии его концентрация может возрастать в сотни раз. Однако именно вследствие этого он не может быть высокоспецифичным для ОИИ, что находит подтверждение в литературе [23,24,25].…”
