RESUMENLos diagnósticos y tratamientos pulpares que fueron realizados en los pacientes del pregrado de la Clínica Estomatológica Central de la Facultad de Estomatología Roberto Beltrán Neira de la Universidad Peruana Cayetano Heredia durante el período 2003 al 2004, se relacionaron según los indicadores de la demanda. Se encontró que la mayor frecuencia de tratamientos pulpares fueron las pulpectomías anteriores (50,2%), seguidas de las pulpectomías en molares (24,8%) y por ultimo las pulpectomías en premolares (24,7%). Los diagnósticos pulpares mas frecuentemente hallados correspondieron a pulpitis aguda (45,1%), pulpa vital (27,4%), encontrándose mayor tratamiento en el sexo femenino. IntroducciónEl profesional deberá constantemente decidir si la pulpa ha sido dañada en forma reversible o irreversible, para así poder brindarle un tratamiento adecuado al paciente. El diagnóstico correcto es el procedimiento de aceptar un paciente, reconocer que tiene un problema, determinar la causa de este problema y desarrollar un plan de tratamiento que ayude a resolverlo, brindándole atención en salud dental. Para evaluar la calidad de la atención en salud se utilizan las variables del contexto (normas vigentes), demanda (estadística de los pacientes), oferta (estructura hospitalaria), procesos (administrativos y atención médica), resultados (producción del hospital) e impacto (costo-efecto de la atención). Los indicadores de la demanda son edad, sexo, grado de instrucción, procedencia, ocupación y mes de atención del paciente, los que determinarán tanto el tipo como la necesidad de tratamiento (1-15).El conocimiento del estado de la pulpa dentaria es imprescindible para elaborar y plantear un adecuado tratamiento. En la endodoncia este procedimiento es crucial por lo que exige todo nuestro esfuerzo y destreza, considerándose que estamos decidiendo sobre la vida o muerte de un tejido tan noble como es el tejido pulpar (16)(17)(18)(19)(20).Estudios determinaron que el mayor número de tratamientos de conductos se ubicó a los 36 años, no hallando diferencia significativa entre las edades según el genero, también se planteó que las mujeres reciben terapia de canales con mayor frecuencia, debido a que disponen de mas tiempo para el tratamiento (21,22). Con respecto al genero, diversos estudios hallaron que en su mayoría pertenecen al genero femenino (23-29), encontraron que la mayor cantidad de pacientes correspondían a estudiantes, amas de casa y en su mayoría eran de sexo femenino entre las edades de 20 a 35 años (30-41).

Core carrier techniques are unique among the various root canal filling techniques for delivering and compacting gutta-percha in the prepared root canal system. Thermafil (TF), considered the major core carrier device, is provided as an obturator consisting of a master core coated with thermoplasticized gutta-percha. We have devised a thermoplasticized gutta-percha filling technique using a polypropylene core, FlexPoint ® NEO (FP), which was developed as a canal filling material that can be sterilized in an autoclave. Therefore, FP can be coated onto thermoplasticized gutta-percha and inserted into the prepared canal as a core carrier. The FP core carrier technique offers many advantages over the TF system: the core can be tested in the root canal and verified radiographically; the core can be adjusted to fit and surplus material easily removed; furthermore the core can be easily removed for retreatment. The clinical procedure of the FP core carrier technique is simple, and similar that with the TF system. Thermoplasticized gutta-percha in a syringe is heated in an oven and extruded onto the FP core carrier after a trial insertion. The FP core carrier is inserted into the root canal to the working length. Excess FP is then removed with a red-hot plastic instrument at the orifice of the root canal. The FP core carrier technique incorporates the clinical advantages of the existing TF system while minimizing the disadvantages. Hence the FP core carrier technique is very useful in clinical practice. This paper describes the FP core carrier technique as a new core based method.

A tooth with a straight root canal is an exception rather than being normal because most teeth show some curvature of the canal. The goal of quality endodontic therapy has remained the same from the beginning.Removal of pulpal tissues with proper cleaning and shaping, followed by an obturation system and coronal seal, will satisfy both mechanical and biological objectives.(1) As clinicians we need to appreciate each of these aspects and know that our therapy's success is dictated by the weakest element of our treatment. One area that has the potential for improvement is our ability to accurately instrument root canal systems in a manner that maintains the original path of curvature in both significant and multiple curvature systems. Presence of curvature may cause difficulty in root canal instrumentation. The final result of instrumentation of curved canals may be influenced by several factors such as flexibility and diameter of the endodontic instruments, instrumentation techniques, location of the foraminal opening and the hardness of dentin.Faili notice canal curvature before treatment can lead to preparation errors such as apical zips, perforations, canal blockages, or instrument separation ,which can leave the canal unprepared and lead to continued pathology compromising the outcome of treatment.(2) In the treatment of curved canals,proper instrumentation helps in achieving true success. These include small stainless steel hand files, nickel hand files, and rotary nickel titanium files.
