Het aanzien van het beroep huisarts neemt af, opleidingsplaatsen blijven leeg, prominente Nederlanders propageren dat de huisartsgeneeskunde afgeschaft kan worden en wij gingen onszelf op het NHG-congres deze week ontwikkelen in onderwijs en onderzoek. Is dat geen regelrechte suı¨cidepoging in een tijd waarin sommigen stellen dat we de ondergang van ons vak alleen kunnen tegengaan door de patie¨nt en niemand anders, in ons denken centraal te stellen? Vraaggestuurde zorg is waar we naartoe zouden moeten; de patie¨nt als consument stelt eisen aan de geboden waar. Wat voor goeds mag de patie¨nt verwachten van de betrokkenheid van huisartsen bij onderzoek en onderwijs?Klinisch onderzoek en zeker huisartsgeneeskundig onderzoek zijn vanuit maatschappelijke optiek vooral relevant en goed als zij informatie opleveren over de optimale behandeling en begeleiding van de patie¨nt. Je kunt er als wetenschappelijk opgeleide huisarts niet omheen. Onderzoek is van groot belang voor de huisartsgeneeskunde, de ontwikkeling van het vak en voor de kwaliteit van de huisarts, stellen Bakkenist en Assendelft in dit nummer.1 Zij reiken een lijst aan die behulpzaam kan zijn bij het beslissen om al dan niet mee te doen aan wetenschappelijk onderzoek. Relevantie voor de patie¨nt is daarbij een belangrijk criterium. Onduidelijk blijft in hun lijst hoe we moeten beoordelen of die relevante vraag met het onderzoek wel beantwoord kan worden. Evenmin is duidelijk hoe je nu precies kunt beoordelen of onderzoek relevant is voor de patie¨nt. Hoe zit het met fundamenteel onderzoek, is dat relevant? Voor welke patie¨nt? Op welke termijn? Lastige vragen, vooral als je bedenkt dat er zelden op grond van e´e´n onderzoek harde conclusies te trekken zijn over de beste diagnostische test en de beste behandeling en dat de toepasbaarheid van inzichten uit fundamenteel onderzoek soms jaren op zich laat wachten.Duidelijk is dat niet alleen onderzoek, maar ook onderwijs ten goede moet komen aan de patie¨nt: de dokter moet communicatief vaardig zijn, goed kunnen luisteren naar de wensen van de patie¨nt en betrokken zijn bij zijn leed. Zij moet ook op de hoogte zijn van de nieuwste inzichten en in staat zijn nieuwe vaardigheden snel onder de knie te krijgen. De impliciete verwachting is dat dit alles ook resulteert in een betere kwaliteit van leven, minder morbiditeit en misschien zelfs minder ontijdige mortaliteit.Aantonen dat onderwijs effect heeft, is erg lastig. Er zijn zeker zoveel factoren op de student of dokter en hun leerrendement van invloed als op de patie¨nt en zijn gezondheid. Veel van die factoren kunnen we maar marginaal of helemaal niet beı¨nvloeden.Er wordt hard gewerkt aan de vernieuwing (lees verbetering) van de opleiding. Van alleen maar aanleren van kennis en vaardigheden stappen we af; het gaat nu om vakbekwaamheid en natuurlijk uiteindelijk om de performance: hoe doet de dokter het in de praktijk? Boendermaker onderzocht welke kenmerken een goede opleider moet hebben.2 Als belangrijkste kenmerken kwamen naar voren: zelfinzicht, flexibilite...
