Priorities for Screening and Treatment of Latent Tuberculosis Infection in the United States

Abstract: Rationale: To improve the effectiveness of tuberculosis (TB) control programs in the United States by identifying cost-effective priorities for screening for latent tuberculosis infection (LTBI). Objectives: To estimate the cost-effectiveness of LTBI screening using the tuberculin skin test (TST) and interferon-g release assays (IGRAs). Methods: A Markov model of screening for LTBI with TST and IGRA in risk-groups considered in current LTBI screening guidelines. Measurements and Main Results: In all risk-group… Show more

“…This is in line with a recent review indicating that use of the QFT test, rather than the TST, is currently recommended for contact tracing and as a screening test for HCW with LTBI in low-incidence countries and for BCGvaccinated individuals (6). In addition, for HCW, QFT screening only is more cost-effective than TST screening (7,12). Our approach to improving the diagnosis of LTBI consisted of increasing the performance of the current IGRA based on the detection of cytokines in addition to IFN-␥.…”

“…9 Furthermore, people with HIV who are exposed to a case of infectious TB are at very high risk to progress to TB disease; therefore, they are the highest priority group to target for latent TB treatment after exposure. 10 As such, HIV testing is recommended for all TB contacts, and HIV-infected contacts are considered high priority for evaluation and latent TB treatment. 11 HIV testing in the setting of TB contact investigations provides a unique opportunity to test people who may not otherwise access health care and to engage people who test positive in HIV care.…”

“…The incidence rate of TB in San Francisco showed a significant decrease in all populations during the study period. This reduction reflects in part on the success of TB prevention and control measures such as contact investigation and treatment of contacts with latent TB infection (LTBI), targeted testing and treatment of foreign-born individuals, intensified screening and treatment of visa applicants in their countries of origin (8,22), and dedicated programs to intensify prevention efforts among the Chinese and homeless populations (J. A. Grinsdale, personal communication).…”
