Dear Sir, We read with interest the article entitled "Bilateral plate fixation for type C distal humerus fractures: experience at a single institution" by Shao-hua Li et al. and congratulate the authors for their work [1].In the surgical technique, the authors converted type C fractures into type A fractures after passing intercondylar K-wire, but the authors did not mention how the compression between the fragments was achieved. We could determine from the X-ray that compression screws were passed through the plate holes but are curious as to whether the same technique was followed in all the patients [2].The authors used 90-90 plating for fixation of type C distal humeral fractures. However, principle-based parallel plating for intercondylar fractures enhances fixation in the distal fragments with structural stability at the supracondylar level [2,3]. The 90-90 plate fixation is not a good option for type C fractures, especially in the presence of comminution, osteoporosis and bone loss [2][3][4]. If the authors had compared the two techniques it would be a significant addition to the literature. Also, the authors did not mention the management of concomitant fractures of the distal end radius (three cases in their study). We would like to know if they were fixed or treated conservatively.The authors did not discuss the postoperative mobilisation protocol they followed. These complex fractures require early joint mobilisation. At one point the authors did mention a long period of immobilisation with the TRAP (triceps reflecting anconeus pedicle) approach. We would like to disagree because, on the contrary, it has been repeatedly published that the TRAP approach allows early mobilisation [3,4].There was a 13% incidence of heterotopic ossification in this study. How were these patients treated and what were the long term outcomes?The authors used tension band wiring with K-wires to fix olecronon osteotomy, but have attributed the delayed healing of the olecranon osteotomy site to early mobilisation. This is contrary to the principle of tension band wiring and articular reconstructions [5].

ÖZET AMAÇ: Bu çalışmanın amacı humerus alt uç eklemi ilgilendiren parçalı kırıklı hastalara uyguladığımız cerrahi tedavilerdeki 90° ve 180° açılı çift plak uygulamalarının sonuçları değerlendirilerek birbirlerine olan üstünlüklerini saptamaktır. GEREÇ VE YÖNTEM: Kliniğimizde 2009Ocak ile 2013 Ocak tarihleri arasında AO sınıflandırmasına göre tip C erişkin distal humerus parçalı kırığı tanısıyla cerrahi tedavi uygulanan, en az 6 ay izleme süresi olan 17 hasta, geriye dönük olarak değerlendirildi. Hastaların poliklinik kontrollerinde ön-arka ve yan grafiler ile radyolojik değerlendirmeleri ve Mayo Dirsek Performans Skorlama sistemi kullanı-larak dirsek işlevleri gözden geçirildi. BULGULAR:Olguların 14'ü (%82.3) erkek, üçü (%17.7) kadın olup yaş ortalaması (22-65) 40.5 yıl idi. AO/ASIF humerus distal uç kırık sınıflandırmasına göre; kırıkların üçü (%17.7) tip C1, dokuzu (%52.9) tip C2, beşi (%29.4) tip C3 kırık şeklindeydi. Bu kırıkların altısı (%35.3) açık olup, bunlarda Gustillo-Anderson'a göre dördü (%23.5) tip 1, ikisi (%11.7) tip 2 idi. Onbir hastada (%64.7) 90°, altı hastada ise (%36.3) 180° açılarla çift plak kullanıldı. Tüm hastalara chevron osteotomisi uygulandı. Hastalar ortalama (6-52) 25.6 ay takip edildi. Hastalarımızın en son kontrollerinde yapılan değerlendirmelerinde ortalama dirsek fleksiyonunun 105º (dağılım 85º-130º), ortalama ekstansiyon kaybının 10º (dağılım 0º-20º) olduğu saptandı. Mayo dirsek performans skorlama sistemine göre; 12 (%70.5) çok iyi, beş (%29.5) iyi sonuç elde edildiği görüldü. Doksan ve 180 derece plak yerleştirilen olguların klinik sonuçları arasında istatistiksel fark saptanmadı (p=0.169). SONUÇ:Erişkin distal humerus C tipi kırıklarda plakların hangi açılarla yerleştirileceğine ameliyat öncesi dönemde çekilen grafiler ve operasyon sırasında cerrahın seçimi neticesinde karar verilmelidir. Özellikle lateral kolondaki parçalanmanın plakların hangi açıyla uygulanacağını belirtmesi açısından önemi vardır. Lateral kolonda parçalanma yoksa 90° açıyla uygulanan çift plak eğer parçalanma varsa 180° açıyla uygulanan plaklama sonuçlar açısından yeterli olmaktadır.Anahtar sözcükler: Çift plak yöntemi ile açık redüksiyon; humerus distal kırıkları; olekranon osteotomisi. GİRİŞSon 20 yıldır, dirsek anatomisi ile cerrahi yaklaşımların daha iyi anlaşılması ve küçük kırık fragmanlarının bile tespitine imkan tanıyan tespit materyallerinin kullanıma girmesiyle cerrahi tedavi literatürde artan sıklıkta önerilmektedir. [1][2][3][4] Erişkin distal humerus kırıklarının nadir görülmeleri ve bölge-nin karmaşık anatomisi nedeniyle bu tip kırıkların tedavisinde kesin bir protokol geliştirilememiştir. SUMMARY -Th e aim of the study was to assess the infl uence of gap shape on biomechanical results in extra-articular distal humeral fracture: with contact on the posterior part (by anterior gap) and contact on ulnar column (by radial gap). Th e goal was to examine if and to what extent did displacements decrease in comparison with previously examined parallel gap without bony contact. Th e fi nite element analysis on the three diff erent plate constructs was performed, i.e. parallel, perpendicular and newly designed Y shape plate were considered. Displacements were measured on articular surface and gap point. Th e most visible decrease of maximum displacements in the distal part of the model was detected in the Y plate model with axial loading: in case of anterior gap 58.5% and especially at radially formed gap 60.9%. Similarly, at axial loading, displacement at the analyzed point on fracture gap most signifi cantly decreased in Y plate model (by 49.4%) at posterior bony contact. Moreover, the latter showed displacement decrease by 68.5% at ulnar bone contact. Furthermore, if a longer radial plate than the ulnar one was used, varus stress could have been avoided. Study results suggested that suffi cient stability could be ensured with the newly designed Y shape plate.
