Objective: The primary aim of this study was to provide a review of the types and frequency of orthodontic brackets, molar bands and orthodontic auxiliaries used for patients undergoing orthognathic surgery. The secondary aim was to evaluate the risk of failure of these items during orthognathic surgery. Methods: From three Dutch hospitals, 124 adult patients were included in this prospective cohort study. Five independent researchers collected the data during surgery using a specifically created data extraction form. The type of surgery, surgeon, orthodontist and type of orthodontic bracket, molar band or auxiliary were noted for each tooth. To evaluate their failure risk, the following variables were noted: failure and site; and type and cause of failure. Results: Stainless-steel brackets were the most frequently used bracket type seen in patients undergoing orthognathic surgery. Ceramic brackets were seen in 24.2% of the cases and were only applied in the anterior region. Molar bands were present in 58.9% of the patients and mostly with bands on the first molars in combination with bonded tubes on the second molars. In 32.2% of all cases, one or more failures were noted. One-third of all failures were described as detachment of the molar tube on the most posterior molar. Kobayashi ligatures and powerpins showed the highest risk of failure (95% confidence interval [CI] = 1.91–7.15). No significant difference in failure rate was found between stainless-steel brackets, molar bands (95% CI = 0.08–1.43) and ceramic brackets (95% CI = 0.14–1.45). Conclusion: Stainless-steel brackets, ceramic brackets, molar bands and surgical hooks are suitable for orthognathic cases. Kobayashi ligatures and powerpins had a significantly higher risk of failure so are not recommended for temporary intraoperative maxillomandibular fixation (TIO-MMF).

Background/purpose To assess the usefulness of plastic bracket primer (PBP) for improving the bond strength of plastic brackets (PBs) using three types of orthodontic brackets, including PBs, metal brackets (MBs), and ceramic brackets (CBs). Materials and methods A total of 162 premolars were gathered and divided equally into six groups of 27. Three groups were tested with the application of PBP (PB+, MB+, and CB+), and three groups were tested without primer (groups PB-, MB-, and CB-). All the groups were bonded using BeautiOrtho Bond II self-etching adhesive. The shear bond strength (SBS) was measured and the bond failure mode was evaluated using the adhesive remnant index after debonding. Results There were significant differences in the mean SBS between groups PB-, MB and CB-, between PB+ and CB+, and between MB+ and CB+. Group PB + had a significantly higher mean SBS than group PB-. The occurrence of bond failure at the enamel and adhesive interface was more frequent in groups PB+ and CB- than in group PB-; and in groups PB+ and CB + than in group MB+. Conclusion Plastic bracket primer can increase the bond strength of PBs to the level of metal brackets, but not to the level of ceramic brackets.

Fixed orthodontic therapies include several procedures that can affect the enamel surface. The aim of this study was to assess the action of composite resin on the surface of the tooth through variation of enamel changes after debonding metal and ceramic brackets, by means of scanning electron microscopy. An in vitro study was conducted on 48 human premolar specimens, which were extracted within a period of two months for orthodontic purposes. On half of them, metal brackets were bonded, and on the other half, ceramic brackets (Al2O3) were bonded, using light cure adhesive paste and a two-step, etch-and-bonding technique. The brackets were debonded after 24 h using a straight debonding plier. The adhesive remnant index (ARI) was determined by visual observation of the specimen. Post-debonding scans were aligned with the baseline, and the surfaces’ changes were quantified. A quantitative analysis was made on the debonded brackets to determine the presence or absence of enamel on the base pad. Evaluation of pre-bonded and post-clean-up enamel surface revealed no crack and increased roughness in both ceramic and metal brackets, which was higher for the ceramic ones. The enameled band (perikymata), artificial caries, or the superficial fissures revealed in the pretreatment stage were replaced with the loss of the prismatic structure and the presence of remnant adhesive. No enamel substance was found on the base pad. The ARI tooth was higher for metallic brackets than for ceramic ones. Metallic brackets and ceramic brackets have undergone mechanical changes by showing fractures in their structure. According to our present investigation, we can conclude that the adhesive composite resin is safe for use on both metal and ceramic brackets in orthodontic treatments, with no iatrogenic enamel damages.
