Primary vitrectomy versus conventional retinal detachment surgery in phakic rhegmatogenous retinal detachment

Abstract: Although primary vitrectomy can achieve anatomical and functional success rates comparable with those achieved by scleral buckling in uncomplicated forms of phakic RRD, the major drawback of the procedure is the high incidence of postoperative cataract formation. Moreover, visual rehabilitation takes place earlier with scleral buckling than with vitrectomy. Scleral buckling should thus be used as the primary surgical modality in the treatment of uncomplicated RRD where the media are sufficiently clear.

“…The anatomical success rate noted in this study is comparable to those of recent prospective studies using 20-G PPV alone (without scleral buckling) in pseudophakic (single-operation anatomic success rate between 62 and 100% [3-5, 7-10, 32, 33] or phakic patients (64-80%) [4,5]. Our results show that 23-G TSV with retinopexy and gas tamponade is also an effective technique in eyes without severe PVR, providing a single-operation anatomic success rate of 77%.…”

“…(RD), especially in pseudophakic eyes, with [1][2][3][4] or without [3][4][5][6][7][8][9][10] scleral buckling. A 23-gauge (23-G) sutureless transconjunctival system was initially reported by Hilton in 1995 (ARVO meeting, known as the Visitrec Vitrectomy Unit) [11] with a two-sclerotomy technique and then developed by Eckardt in 2005 [12] with a three-way PPV technique.…”
