Primary Unilateral Cleft Nose Correction: A 10-Year Follow-up

Abstract: Primary nasal correction at the time of rotation-advancement of the lip has been rendered practical by developing a symmetrical nasal platform by presurgical orthodontics and a gingivoperiosteoplasty. The nasal correction involves unilateral columella lengthening, alar cartilage positioning, and alar base cinching to present a nose within normal limits. This is accomplished before the age of recall, and by avoiding retention of the nasal deformity through childhood, it by-passes the ridicule that invariably ac… Show more

“…Particularly in pediatric cases, a septorhinoplasty is not performed based on growth and development, although only a medial mobilization is permitted. Only in a limited number of severe cases is the fixation of cartilages permitted [15,17,18].…”

“…Some surgeons have applied presurgical orthodontics and the lip adhesion technique for alignment of the nasal platform before primary nasal repair. 5 In primary lip correction, appropriate repositioning of the alar base is important, because the alar correction is easiest in primary correction with or without nasal correction. However, a postoperative "retruded" appearance of the ala (Fig.…”

“…Numerous techniques have been described for correction of nasal deformity for achieving reasonable nasal shape [3][4][5][6][7][8][9][10][11] ; nevertheless, secondary nasal correction is still warranted in many cases because further growth of the nose often leads to deterioration of symmetry. Apart from alar cartilage sculpturing procedures, attention to the nasal septum is of paramount importance for improving the architectural support and aesthetic appearance of the nose (especially the tip area) in addition to providing undisturbed breathing function.…”
