Primary Small Bowel Volvulus: An Unusual Cause of Small Bowel Obstruction

Abstract: We present the case of a 78-year-old female who presented to the emergency department with small bowel obstruction in a virgin abdomen. Although the patient did not have peritonism and biochemical investigations did not reveal alarming features of ischemia, an abdominal computed tomography (CT) scan was suggestive of small bowel volvulus (SBV), and operative exploration was pursued. No obvious cause was identified aside from hard stools throughout the colon and a diagnosis of primary SBV was determined. She wa… Show more

“…The annual incidence of SBV is 3.5 cases [14]. The SBV accounts for 1 to 25 % of small bowel obstruction (SBO) [1], [5], [7], [11], [15] and 1.7 to 27% of all intestinal obstruction (IO) [4], [13] - [17].…”

“…Obstruction and strangulation frequently crop up and progress quickly to gangrene as the mesenteric twist tends to be very tight leading to early vascular involvement [20]. The sequelae of that will steer to intestinal obstruction, bowel gangrene, perforation peritonitis [1], [7]. If an extensive segment of the loop is involved, then a large volume of blood and plasma are extravasated into the intestinal wall and lumen.…”
