Primary pulmonary cryptococcosis

Abstract: SUMMARYHere, we report a case of primary cryptococcal pneumonia in a 25-year-old woman who presented with several weeks' history of cough, dyspnoea and night sweating. These symptoms started in the third trimester of her pregnancy. She was being treated for infertility and got pregnant with in vitro fertilisation. On chest imaging, there were bilateral air space consolidation and cavitary lesions. Fungal pulmonary infection was diagnosed after surgical lung biopsy. She received fluconazole 400 mg per day orall… Show more

“…Although some clinical cases of cryptococcosis and a few environmental studies were reported in Iran, C. neoformans var. grubii was only detected by Badali et al, and Pakshir et al, using molecular methods (21)(22)(23)(24)(36)(37)(38). Cryptococcosis was reported from different parts of the world, including India (39), Italy (40), Brazil (41), and China (7,42).…”

“…Despite the development of molecular methods, very limited studies have been conducted on the diversity of Cryptococcus species in Iran. Most clinical available reports in Iran are as case reports (21)(22)(23), and environmental epidemiological studies are limited to formal identification. The present study aimed to isolate and identify Cryptococcus species from pigeon guano based on molecular methods in Ahvaz, Iran and investigate the important virulence factors in the isolated species.…”

“…For the patient in question, there was remission of symptoms and significant radiological improvement with the use of fluconazole 400 mg once daily for 18 months. Fluconazole therapy is also well established; the difference lies in the duration of drug use [ 14 ]. Amphotericin B is the preferred initial treatment for cryptococcal meningoencephalitis, disseminated disease or severe pulmonary cryptococcosis in pregnant patients.…”

“…Therefore, there is a need for other non-invasive rapid diagnostic tools. Any serum biological marker that could strongly predict SBP may obviate the need for paracentesis while increasing the patient's chance of survival by expediting the diagnosis and treatment of SBP (13) . C-reactive protein (CRP) is an acute phase reactant which binds to different substrates.…”
