Primary Pituitary Carcinoma with Spinal Cord Metastasis

Abstract: The authors report a rare case of non-functioning pituitary carcinoma with spinal cord metastasis. A 37-year-old female presented with a 2-month history of right retro-orbital ache and vomiting. She had a pituitary adenoma removed 3 years prior to admission. Neuroradiologically, a mass lesion was demonstrated in the right middle cranial fossa. The tumor was removed through craniotomy and was histologically diagnosed as pituitary carcinoma. One week after the operation, tetraplegia developed and CT scans demons… Show more

“…Because many of the latter were described prior to the routine measurement of PRL or the routine use of immunochemistry, it is likely that the true frequency of PRL cell carcinomas has been grossly underestimated. That this is the case is also suggested by the relative lack of "nonfunctioning" carcinomas in more recent reports 37, 38, 45. Summarizing the literature, the most common endocrinologically functional subtype, PRL cell carcinoma, was represented by 21 cases (47% of functioning tumors), followed by 18 ACTH‐producing tumors (40% of functioning tumors).…”

“…With the addition of our 15 cases, 3 of which were previously published, a total of 64 cases of pituitary carcinoma have been reported in the literature published in English to date 1‐51. Forty‐eight (74%) were hormone‐producing, whereas the remaining seventeen (26%) were considered endocrinologically "nonfunctioning."…”

“…According to previous reports, most patients with pituitary adenoma who experience recurrences (including intracranial and spinal metastases) have undergone multiple previous surgeries and have received prolonged radiotherapy. 1 , 2 , 5 , 6 , 9 , 11 , 13 , 14 , 16 , 21 , 22 , 24) Further, cerebral spinal fluid (CSF) leakage following transsphenoidal surgery may increase the risk of subsequent subarachnoid dissemination of the tumor. 16) …”

“…A total of 45 of 46 (97.8%) published cases described utilizing radiation therapy prior to metastatic progression were reviewed. [15814161719232527293137394144454748505156586062] Eight patients had undergone stereotactic radiosurgery (Mean: 1.13 treatments) prior to progression. Forty patients underwent external radiation therapy (Mean: 1.11 treatments).…”
