Primary Peritoneal Mullerian Adenosarcoma with Sarcomatous Overgrowth Associated with Endometriosis: A Case Report

Abstract: We report a primary peritoneal mullerian adenosarcoma with sarcomatous overgrowth associated with endometriosis in a 50-year-old female. The tumor formed multiple peritoneal masses occupying the pelvis and subdiaphragmatic space. Histologically, the tumor was composed of benign-appearing mullerian glands surrounded by a sarcomatous stroma. In addition, one perisplenic mass was largely devoid of the epithelial component and was of higher grade than the remaining lesions. Multiple foci of endometriosis were asso… Show more

“…To limit the scope to extragenital cases, we excluded those cases originating in the uterus, cervix, adnexa, and vagina. In addition to the 3 aforementioned adenosarcoma cases associated with gastrointestinal/hepatic endometriosis, we identified 18 cases of extragenital adenosarcoma for which treatment and follow-up information was reported [4-6, 16, 17]. We identified five previous cases associated with endometriosis.…”

“…In 2 of these 5 patients, sarcomatous overgrowth was described. In the first case, reported by Dincer, a patient with a long history of recurrent endometriomas developed multiple pelvic masses and a large peri-splenic mass [17]. The pelvic masses contained multiple foci of endometriosis and low-grade sarcoma, while the perisplenic mass contained a high-grade pure sarcoma accounting for 50% of its mass.…”

“…However, its association with endometriosis has been suggested [2]. Occurrence of carcinosarcoma with previous radiation exposure or post radiation therapy has also been reported [1,3,5].…”

“…It occurs mainly in the uterus of postmenopausal women but can occur in adolescents and young adults [2]. Extrauterine locations such as ovaries, cervix, vagina, peritoneum and Pouch of Douglas also had been reported [3][4][5][6]. To date, this neoplasm has been reported in only 10 young adult and adolescent girls.…”
