Primary pelvic hydatid cyst presenting with obstructive uropathy and renal failure

Abstract: Summary:Primary pelvic hydatid cyst is a rare entity. Pelvic hydatid cysts usually present with pressure symptoms involving adjacent organs (bladder and rectum usually). A case of primary pelvic hydatid cyst presenting with obstructive uropathy leading to chronic renal failure is presented. A combination of preoperative albendazole therapy of 1.2 g/day for 8-12 weeks and surgical excision were effective in alleviating the symptoms and improving the renal function.

“…The incidence of the disease in pregnancy has been found as low as 1 in 20,000–30,000 [4], and usually it is asymptomatic but it was reported to cause anaphylaxis when ruptured intra-abdominally [19] and obstructed labour [20]. Most intact lung cysts are discovered incidentally on chest X-ray which probably was delayed in our patient because she was pregnant.…”

“…It is not clear how the hydatid scolex reaches the pelvis, but it has been postulated that it passes through the portal system into the liver and retroperitoneal lymphatics, or directly via the gastrointestinal tract. 2,7 Pelvic hydatid cysts usually present as a nonspecific mass with compression of adjacent organs, e.g., the rectum and urinary bladder and uterus. 2,8,16 Urinary frequency and tenesmus were major symptoms in our case, but the unique characteristic was extension of pelvic lesion to the left perianal and gluteal regions as a soft, painless mass.…”

“…Symptoms depend on the size and location of the cyst. Pelvic hydatid cysts usually present as a nonspecific mass with pressure effects on urinary bladder and rectum [8]. Patients may present with pain, frequency, urinary retention and pain on micturition, constipation, weight loss and renal insufficiency.…”
