Primary omental pregnancy

Abstract: We present a case of primary omental ectopic pregnancy in a 31-year-old woman which was discovered intraoperatively during a diagnostic laparoscopy and subsequently removed via mini-laparotomy. We emphasise the rarity of this diagnosis, and the importance of careful inspection of the abdominal cavity including the omentum should an ectopic pregnancy be suspected when bilateral fallopian tubes and ovaries appear normal during surgical exploration.

“…Laparotomy with excision of the embryo has been the classic management for abdominal pregnancy. 4 However, with its widespread availability, laparoscopy should be the modality of choice, especially when the patient is haemodynamically stable, as in our case, and the required expertise is available. Laparoscopic management is associated with fewer morbidities, reduced intraoperative blood loss and a shorter hospital stay.…”

“…Omentum pregnancy is a rare type of abdominal pregnancy, which is itself a rare presentation. Pregnancy of the omentum is a rare subset of abdominal pregnancy that can only be diagnosed during surgery, 1 25 as the location of the ectopic embryo can only be determined at this time. Ectopic pregnancies can be dangerous and may cause a large amount of abdominal bleeding.…”

“…Despite methotrexate therapy, symptoms increased and she underwent laparoscopy for management of the presumed ruptured ectopic pregnancy. Abdominal pregnancy is rare, occurring in approximately 0.34-1.3% of all ectopic pregnancies 1,2 , with only 9% found in the omentum 2,3 . Maternal mortality from abdominal pregnancy is 7.7 times higher than tubal ectopic pregnancy 4 .…”
