Primary monophasic synovial sarcoma of the pleura: diagnosis and treatment

Abstract: Synovial sarcomas are uncommon soft tissue malignancies that usually affect the extremities in the vicinity of large joints. The recognition of this tumor in an unexpected site, such as the pleura, is often difficult and the monophasic variant of synovial sarcoma is often mistaken for some other spindle cell tumor. In this report, we describe a very rare case of primary monophasic synovial sarcoma of the pleura treated with radical surgery.

“…In the absence of typical clinical manifestations and signs, the early diagnosis of the disease often depends on the results of imaging examinations. CT scans of mediastinal SS showed a soft tissue density shadow, lobulated appearance, clear edge, homogeneous enhancement, and liquefaction necrosis in the tumor, but these findings are often difficult to differentiate from those of solitary fibroma, leiomyosarcoma and rhabdomyosarcoma (23)(24)(25). In our study, 2 cases of mediastinal SS were not biopsied before surgery, one of which was submitted to an MDT meeting for discussion before surgery, and SS was considered.…”

“…However, other groups have applied similar treatment to that used in the present study in synovial sarcomas occurring at other sites (12). Primary pleural synovial sarcoma has a high recurrence rate compared to other sarcomas.…”

“…However, Baccari-Ezzine et al (12) reported a case of intracardiac metastasis at the apex of the right ventricle from primary SS in the lungs. Although primary pleural SS is rare, this condition has been documented (13,14); therefore, it is possible that our patient had primary pleural SS with intravascular metastasis.…”
