Primary malignant tumors of the thyroid gland.A clinicopathologic study of 254 cases

Abstract: Of 254 primary malignant tumors of the thyroid gland diagnosed in the pathology department of the University of Cape Town, 212 (83.5%) were differentiated carcinomas. In this differentiated group, tumors composed exclusively of papillary structures exhibited the most favorable prognosis; local recurrences and/or spread were noted in tumors with a mixed papillary and follicular pattern and, more commonly association with 21 tumors (8.3%) but an associated multinodular goitre was found in only nine patients (3.5… Show more

“…Surgery is the mainstay of treatment for HCN. Most experienced surgeons recommend aggressive surgical approach for MHCN and lobectomy or hemithyroidectomy for BHCN 7,23 . In our institution most of malignant HCN have had TT (whenever possible) plus cervical lymph node dissection, if the jugular lymph node sampling was positive.…”

“…The reported series have indicated highly variable prevalence; this may be because of a lack of adherence to strict criteria in excluding non‐neoplastic Hurthle cell lesions 1,10,21 . Hurthle cells can commonly be present in a variety of thyroid pathology, including Hashimoto’s thyroiditis, nodular goitre and differentiated TC 1,2,22,23 . HCN may constitute most of follicular carcinomas and Hurthle cell variant papillary carcinoma with higher recurrence and mortality rates have also been described 24,25 …”

“…There was no statistical difference between the two groups in the incidence of associated malignancy, a finding that agrees with similar observations in the literature. l 3 The mean weight of the glands with OPC was 20.70 g * 5.06 (Table 2). …”
