Malignant melanoma (MM) and blue nevi of the uterine cervix are an extremely rare neoplasm, probably derived from embryologic migration of melanocytes from the neural crest. MM displays aggressive behavior with a poor prognosis. We report the case of a 76-year-old postmenopausal woman abnormal vaginal bleeding. She underwent a hysterectomy and bilateral salpingo-oophorectomy with paraaortic-iliac lymphadenectomy. Histopathological and immunohistochemical studies were consistent with the diagnosis of MM and blue nevi in the uterine cervix. Although it is extremely rare, this case suggests that MM of the uterine cervix should be considered in the differential diagnosis of undifferentiated neoplasm. Early diagnosis is essential in order to warrant a better prognosis, although there are no cases of cure described.

PurposeTo investigate the clinical characteristics and prognosis of primary malignant melanoma of the uterine cervix.ResultsThe median age of the patients was 61.2 years (range, 42-78 years). The median overall survival of the patients at stage I, II and III were 39.2 months, 47.8 months and 9.0 months (P=0.574) and the 2-year overall survival for each stage were 80.0%, 50.0% and 0.0% respectively. Twelve (85.7%) patients developed recurrence and eleven patients (78.6%) died. Ten patients received surgery and four patients were treated with chemotherapy and/or radiotherapy. Immunotherapy was administrated to two patients after surgery. The mean survival time of patients with surgery and without surgery were 47.9 vs.7.75 months (P=0.047). Patients received radical hysterectomy had longer survival than patients underwent total hysterectomy (66.8 months vs 19.5 months, P=0.016).MethodsClinical data from 14 patients with primary malignant melanoma of the cervix between January 1981 and December 2014 were reviewed.ConclusionsPatients with primary malignant melanoma of the cervix have a poor prognosis. Radical hysterectomy and pelvic lymphnode dissection may offer better prognosis for stage I and II patients.

Primary malignant melanoma (MM) accounts for 1% of all cancers, and only 3% to 7% of these tumors occur in the female genital tract. Data are limited with respect to the basis for treatment recommendations because of the rarity of MM. The overall prognosis of melanomas of the female genital tract is very poor. Two cases of MM of the female genital tract are presented. The first case is of a 70-year-old female patient who complained of left thigh pain and underwent magnetic resonance imaging that showed cervical cancer with involvement of the vagina, bladder, and parametrium, in addition to multiple bony metastases of the proximal femur, acetabulum, and both iliac bones. The second case is of a 35-year-old female patient who suffered from vaginal bleeding for 5 months, and she was diagnosed as having primary vaginal melanoma. The patient underwent radical surgery and two additional surgeries because of recurrence of cancer in both inguinal areas. After surgery, the patient received adjuvant immunotherapy, radiation therapy, and chemotherapy. In both the aforementioned cases, the pathologic diagnosis was made after immunohistochemical analysis, i.e., the tumor cells were stained with HMB-45 and S100, and were found to be positive for both immunostains.
