Primary lymphoma of the pericardium: Report on a „Cured”︁ case and review of the literature

Abstract: This report describes a 62-year-old man with a primary diffuse, immunohistochemically proven B-cell lymphoma of large non-cleaved cell (centroblastic) type of the pericardium. The patient responded completely to systemic chemotherapy and remains free of disease 30 months after diagnosis. The use of non-cardiotoxic drugs in divided doses as initial treatment is emphasized. In addition, the authors reviewed the literature of the last decade regarding the management and outcome of patients with primary cardiac ly… Show more

“…In the reviewed cases, ten patients underwent surgery with no evidence of improvement in survival [3,6,9,15,19,23,27,35,38,39]. Prompt anthracycline-based chemotherapy has been administrated in 51% of the reviewed cases [1,3,6,7,8,9,12,17,19,20,25,26,29,30,31,38,39,41], resulting in 61% of complete remission (mean follow-up times 17 months; range 3-40 months). Radiotherapy was delivered to 17% of patients, alone or combined with chemotherapy, with a further improvement in survival [3,17,25,26,34,38].…”

“…Primary cardiac lymphoma remains asymptomatic until it produces a mass effect when the tumor obstructs cardiac output or causes local invasion, embolization, or systemic manifestations. Chest pain is the most common symptom occurring in 26% [6,10,18,27,28,29,30,31,38], dyspnea in 20% [7,8,9,28,34,36,37], superior vena caval syndrome in 9% [3,15,17] Chest X-ray is not usually helpful for the detection of cardiac tumor. It can reveal non-specific signs such as pleural effusion and/or cardiomegaly.…”

“…La poliquimioterapia ha sido empleada obteniéndose escasas remisiones completas (8)(9)(10)(11). No obstante, en algunos casos en que se ha podido establecer un diagnóstico precoz la poliquimioterapia intensiva ha permitido mejorar el pronóstico (6,7,12). En algún paciente se ha realizado quimioterapia intensiva seguida de trasplante de progenitores hematopoyéticos de sangre perifé-rica, con buena respuesta (13).…”
