Primary intracranial plasma cell granulomas presenting as malignant neoplasms

Abstract: Plasma cell granuloma (PCG) is an uncommon non-neoplastic mass lesion of unknown etiology. It is characterized by a polyclonal proliferation of chronic inflammatory cells, mostly mature plasma and other mononuclear cells. PCGs arising in the central nervous system are particularly rare. We report two additional cases of intracranial PCG exclusively involving the brain parenchyma. A 47 year-old woman, presenting with partial motor seizures and fluent aphasia, underwent complete excision of a well-demarcated, en… Show more

“…Tissue sections in the present patient contained a perivascular infiltrate of lymphocytes and polyclonal plasma cells, which is characteristic of plasma cell granulomas (3,6,7). A slight predominance of κ as compared to λ light chains was noted, and this has also been found in other cases of plasma cell granuloma.…”

“…In the present study, the patient was followed until death, fourteen years following the initial diagnosis. Furthermore, the individual suffered four disease recurrences, adding to the eight reported cases of relapsing intra-axial plasma cell granuloma (3). Treatment of the recurrences consisted of two craniotomies, 3,600-cGy fractionated radiation and two courses of glucocorticoid therapy.…”

“…Actually, the present case was similar to the published cases with tumorlike demyelinating lesions and accompanied by meningeal involvement which was evidently demonstrated on MR images. We speculate that the same radiologic appearance may contribute to the serviceability of image diagnosis for tumor-like demyelinating lesion [14].…”

“…PCG is an uncommon non-neoplastic lesion with the polyclonal proliferation of plasma cells [1]. This disorder is known to occur in numerous sites, with the lung mostly reported [2][3][4][5][6].…”
