Primary hyperparathyroidism. Changing clinical spectrum, prevalence of hypertension, and discriminant analysis of laboratory tests

F. W. Lafferty

Abstract: The clinical spectrum of 100 consecutive cases of surgically proved primary hyperparathyroidism treated from 1974 through 1978 was analyzed. Their laboratory test results were compared with 64 cases of other form of hypercalcemia using multivariate discriminant analysis. The clinical spectrum has dramatically shifted during the past three decades from renal calculi and bone disease to the asymptomatic patient discovered by routine serum chemical analysis. Hypertension was twice as common among hyperparathyroid… Show more

“…Although several common laboratory values such as the chloride phosphate ratio and serum albumin may serve as a guide for the differential diagnosis between PHP and hypercalcemia of malignancy (39,40), our data showed these values were less specific. Incorrect classification remained a significant problem, although 80 percent discrimination could be achieved by the chloride phosphate ratio and serum albumin using logistic regression analysis in our study.…”

“…34 " 37 The association initially was thought to be due to a direct effect of hypercalcemia on blood pressure, a pressor action of parathyroid hormone, or renal damage from antecedent stone disease. 33 The retrospective reports that blood pressure decreased following parathyroidectomy 33 -M supported these possibilities; however, in his prospective assessment Lafferty 36 indicated otherwise. First, he noted that the demographics of this clinical correlation were similar to that of hypertension in our society, that is, there was a greater prevalence among blacks and older persons.…”

“…Clinical presentation of PHPT has changed in recent decades due to the large availability of routine serum calcium measurements, and PHPT is often discovered by chance. The clinical features of PHPT are mostly mild and sometimes totally asymptomatic (10,12); however, an accurate evaluation can reveal some subclinical pathologies including cardiovascular system impairment, one of the most important target systems of PTH action (20,21).…”
