Penile cancer is generally rare, and Squamous cell cancer of the penis is the most common histological type. Sarcoma of the penis has a low incidence, but they tend to grow faster than other penile cancers. One of the rarest types of penile sarcomas is Extra-Skeletal Ewing's Sarcoma (EES). The management of such cases can be challenging, and treatment guidelines do not exist for these rare cases. We present a rare case of EES that has developed in the penis of a young patient in the United Kingdom.

Case Report: Primary Ewing Sarcoma of the Penis With Multiple Metastases

Background: Ewing sarcoma is the second most common malignant bone tumor in children, but it rarely originates from extra-skeletal sites. The commonly involved sites of soft tissue include paravertebral spaces, lower extremities, the pelvis, head, and neck, while primary extra-skeletal Ewing sarcoma (EES) located in the genitals is extremely rare.Case Presentation: We report a young patient who presented to our hospital with a painful erection of the penis and limited motion of the left hip. Magnetic resonance imaging showed a hyperintense mass with invasion of adjacent tissue in the penis and a heterogeneously high signal lesion in the left proximal femur. 18F-fluorodeoxyglucose positron-emission tomography detected widespread metastatic lesions in the bilateral lung and multiple skeletons. An incisional biopsy of the penis was performed; the histopathological findings and EWS gene translocation identified by molecular analysis confirmed the diagnosis of Ewing sarcoma. Subsequently, the punch-biopsy specimen from the left femur showed undifferentiated small round cells, a finding consistent with the microscopic presence of Ewing sarcoma metastasis. However, after the first course of multiagent chemotherapy, the penile mass did not obtain stabilization but instead grew progressively with surface ulceration and multidrug resistant bacteria infection. Despite receiving antibiotics and maximal supportive therapy, the patient died from sepsis and lung metastasis complications in the intensive care unit 2 months later.Conclusion: This case indicates that although EES as a subtype of Ewing sarcoma is rare, it can occur virtually in any soft tissue site, even in the genitals. Therefore, clinicians need to distinguish this entity from other soft tissue sarcomas with rapid progression since early diagnosis and timely treatment of EES are pivotal for a favorable prognosis.

Vychází nejčastěji ze skeletu dlouhých kostí a hrudní stěny. Vzácné je primární extraoseální postižení. Tvoří 11-24 % z celkové incidence, výskyt stoupá s věkem. Popis případu: Předkládáme kazuistiku 57leté pa cientky s lokálně pokročilým tumorem děložního čípku klinického stadia IIB. Na základě histologického a molekulárně-genetického vyšetření s nálezem přestavby EWS-ERG byl dia gnostikován Ewingův sarkom. CT trupu dále odhalilo patologickou pánevní lymfadenopatii a vícečetná plicní ložiska oboustranně, scintigrafi e skeletu neprokázala kostní postižení. Pa cientka absolvovala dvoufázovou intenzivní chemoterapii režimy VIDE (vinkristin, ifosfamid, doxorubicin, etoposid) a VAI (vinkristin, aktinomycin D, ifosfamid). V rámci druhé fáze proběhla konkomitantní radioterapie pánve. Bylo dosaženo kompletní remise onemocnění podle PET/ CT. V rámci konsolidace byla léčba doplněna velkoobjemovým ozářením plic. Závěr: Primární Ewingův sarkom děložního čípku je extrémně vzácné onemocnění. Dosud bylo zaznamenáno pouze 12 případů s průměrným věkem v době dia gnózy 35 let, na rozdíl od naší kazuistiky bez iniciálního průkazu vzdálených metastáz. Léčba byla vedena podle protokolu Ewing 2008 navrženého pro primárně kostní formu Ewingova sarkomu. Aktuálně je pa cientka 18 měsíců po ukončení léčby bez známek onemocnění, dlouhodobé sledování je nezbytné. Primary localization is mostly in the skeleton of long bones and chest wall. Primary extraosseous involvement rarely occurs, incidence increases with age. Case: We present a case report of a 57-year-old patient with locally advanced tumors of the cervix, clinical stage IIB. Due to histological and molecular genetic examination revealing EWS-ERG fusion gene, Ewing's sarcoma was dia gnosed. CT revealed pathological pelvic lymphadenopathy and multiple pulmonary bilateral methastases, scintigraphy did not prove any aff ection of skeleton. The patient underwent a two-stage intensive chemotherapy regimens VIDE (vincristine, ifosfamide, doxorubicin, etoposide) and VAI (vincristine, actinomycin D, ifosfamide). During the second phase, concomitant radiotherapy of pelvis was aplied. According to PET/ CT, complete remission was achieved. Whole-lung irradiation was applied in consolidation of the result. Conclusion: Primary Ewing's sarcoma of the cervix is an extremely rare disease. To our knowledge, only 12 case...
