Introduction: Urethral stricture is "narrowing of the caliber of the urethra caused by the presence of a scar consequent to infection, inflammation or injury". Urethral strictures are difficult to manage. BXO is more resistant to all types of treatment and recurrence is the most common complication. Buccal mucosa dorsal onlay graft urethroplasty is one of the surgical treatment modality for treating BXO related strictures. Objective and Methods: This study is to assess the outcome of stricture urethra in BXO cases treated by single stage buccal mucosa substitution urethroplasty. A Prospective study, total 35 patients underwent surgery for stricture urethra with BXO. Results: 33 patients underwent single stage dorsal onlay buccal mucosa substitution urethroplasty and 2 patients underwent augmented roof strip buccal mucosa urethroplasty Pre and post operative uroflowmetry done for all patients, a significant increase in post-operative uroflowmetry rates observed (P value is less than 0.0001.). Patients were followed up post-operatively at 3, 6, 9, 12 and 36 months intervals out of 35 patients 6 patients had the recurrence. Out of 6 patients, 3 patients underwent one single procedure for recurrent stricture. the success rate is 91.42%. (mean followup period 36.2 months). Conclusion: Buccal mucosa dorsal onlay urethroplasty a good surgical option for BXO patients with less recurrence rate.

Objective To evaluate the long‐term outcomes of dorsal onlay urethroplasty using lingual mucosal graft for repairing urethral strictures associated with genital lichen sclerosis. Methods This study included 36 patients who had lichen sclerosis long anterior urethral strictures that were managed with dorsal onlay urethroplasty using lingual mucosal graft, and were followed up ≥5 years. Preoperatively, we measured the maximum urinary flow rate and the International Prostate Symptom Score, then every 3 months in the first year, and annually thereafter. During follow up, patients with obstructive symptoms were subjected to urethrography and/ or urethroscopy. A successful urethroplasty was defined as normal voiding and no need for further intervention. Results Of the 36 patients, two were lost during the follow up, thus 34 patients were involved in the assessment. After lingual mucosal graft urethroplasty, there were significant improvements in maximum urinary flow rate and International Prostate Symptom Score (P < 0.0001). This improvement was sustained during the 5‐year follow‐up period. The median follow‐up period was 66.5 months (interquartile range 64–70 months). The overall success rate in this study was 88.2%. Postoperative complications that required intervention were reported within the first year in four (11.8%) patients. Oral site complications were mild in the early postoperative period with no long‐term complications. Conclusions Dorsal onlay urethroplasty using lingual mucosal graft is a reliable and durable procedure for repairing lichen sclerosis urethral stricture. It provides a long‐term success rate with few failures occurring within the first year. Lingual mucosal graft harvesting is associated with minor, immediate oral complications, and no long‐term morbidity.

The U.L.T.R.A. measurement scoring system proposed through urethral ultrasonography is a standardized system, which is a repeatable disease assessment tool. The five characteristic factors that were most associated with anterior urethral stricture were summarized. Through this, we can more accurately describe the degree of the disease and refinement grade the complexity of the treatment, and enable imaging technologists and surgeons to establish effective communication channels.
