Primary cutaneous signet ring cell carcinoma expressing cytokeratin 20 immunoreactivity

Abstract: Primary cutaneous signet ring cell carcinoma (PCSRCC) is a very unusual but distinctive clinicopathologic entity that can simulate metastatic adenocarcinomas. It is defined as a diffuse malignant epithelial neoplasia localized in the dermis and subcutis without epidermal involvement, showing variable amounts of signet ring cells, without evidence of visceral adenocarcinoma. We present 2 cases of PCSRCC, which involved eyelids and axilla respectively. Despite thorough systemic workup, primary sources could not … Show more

“…CK 20 might be a helpful marker because its weak expression was reported in 2 cases of primary signet ring cell carcinoma of the eyelid. 91 Recommended panel: p63, CK15, D2-40 Primary adnexal carcinoma: p63+, CK15+, D2-40+ Cutaneous metastases: p63-, CK15-, D2-40-…”

“…The patients usually present with an indurate lesion on 1 eyelid especially the lower eyelid, most of which subsequently develop involvement of the other eyelid of the same affected side, causing a "monocle-like" 16 appearance. Involvement of upper and lower eyelids at the initial presentation is noticed in 9 patients 3,8,10,11,[15][16][17] including this case. There is minimal left to right side predilection (1.2:1).…”

“…Primary cutaneous signet ring cell carcinoma shows intracytoplasmic mucin vacuoles and expresses cytokeratins CAM5.2 and AE1-3, cytokeratin 7, epithelial membrane antigen (EMA), CEA, p63, mucin-1, 16,17 and cytokeratin 20. 18 Cutaneous metastases from signet ring cell carcinoma may be the initial presentation of a gastric or colorectal carcinoma. In both cases, there are intracytoplasmic mucin vacuoles.…”

“…7 In the cutaneous signet ring cell adenocarcinoma, either primary or metastatic, intracytoplasmic vacuoles containing mucin are positive with PAS, PAS-digest, and colloidal iron. 7,[16][17][18] The tumor cells express cytokeratin, and most vacuoles express CEA while vimentin is negative. 7 In the case of signet ring cell melanoma, either primary or metastatic, intracytoplasmic vacuoles are negative with PAS, colloidal iron, cytokeratin, and CEA.…”
