Primary cutaneous melanoma of the scalp: Patterns of recurrence

Abstract: Our results support the hypothesis that primary scalp melanoma represents a unique aggressive subcategory with high rates of in-transit disease and poor disease-related and survival outcomes. There is a need for robust prospective comparative studies to address the significance of resection plane in the management of patients with scalp melanoma.

“…79 Tas when the melanoma has already reached a significant thickness. [80][81][82][83] A variety of factors have been incriminated to account for the high local recurrence. These include increased thickness of the primary lesion, inadequate excision margins due to anatomic constraints and unfavorable histology.…”

“…One probable cause is the late diagnosis, as the scalp region is often covered by hair and hence not easily accessible to self or clinical examination. The first clinical symptoms generally appear when the melanoma has already reached a significant thickness 80‐83 …”

“…23 The improved side-effect profile and reproducibility of results has led to the acceptance of ILI as a safe and effective treatment of unresectable melanoma metastases in Australia, where it remains the standard of care. 24 Within the current study, this possible confounder was largely controlled for by the matching process, although three patients had nonextremity-based disease. As an intralesional agent PV-10 can be selected for such patients, although these treatment sites are known to be associated with worse long-term survival outcomes independent of treatment type.…”

In the PV-10 subgroup, the ORR was 83.4% (CR 25.0%; PR 58.4%) and this response profile is similar to other institutional treatment results and data from the phase II study.…”

“…Primary melanoma of the scalp represents a unique subcategory with an aggressive phenotype characterized by high rates of local, in-transit metastases, and regional recurrence as well as poor disease-free survival outcomes [7]. It's more likely to be rapidly growing, a melanotic and nodular in subtype.…”
