Primary cN0 lip squamous cell carcinoma and elective neck dissection: Systematic review and meta-analysis

Abstract: The results do not prove sufficient to justify elective treatment of the neck in primary cN0 lip SCC and close observation would be a viable option in such cases. © 2014 Wiley Periodicals, Inc. Head Neck 37: 1392-1400, 2015.

“…2,4,5 We believe that these findings provide evidences that can help to differentiate SDSC from sinonasal undifferentiated carcinoma (SNUC), which according to WHO (2017) classification, it is uncertain if SDSC is a distinct entity, being classified within SNUC spectrum. 23 Recently, a series of 12 SMARCB1 (INI-1)-deficient sinonasal adenocarcinomas of high-grade, with variable degrees of glandular differentiation and foci of intracellular or intraluminal mucin, have been reported, 6 expanding the clinicopathologic spectrum of SMARCB1 (INI-1)-deficient tumors. Loss of this latter leads to upregulation of cyclin D1, resulting in cell-cycle progression.…”

“…[6,[11][12][13] Bununla birlikte bölgesel lenf nodu metastazı gelişen hastalarda yaşam süreleri ciddi oranda düşmektedir. [14][15][16] Dudak kanserlerinde en önemli prognostik faktör boyun metastazı, en önemli ölüm nedeni ise kontrol edilemeyen boyun nüksleridir.…”

“…Thus, the gold standard of neck staging is still elective neck dissection (END) and subsequent histopathologic analysis of the surgical specimen . END (of regions I–III) has traditionally been recommended in head and neck SCC when the risk of occult metastases is more than approximately 20% . However, it still carries significant morbidity …”

“…Although SCC of the lip is often considered an oral malignancy, it is clearly less aggressive than intraoral SCC proper: metastases to the regional cervical lymph nodes are not as frequent and usually develop later in the course of disease. A 10% cohort subpopulation remains evident, comprising those who will develop regional metastatic disease with a much poorer prognosis: their survival is reduced to approximately 30% to 70% . This is the most important prognostic factor in SCC of the lip, and head and neck SCC in general.…”
