Priapism Caused by Infection and an Inflammatory Process in the Pelvic Region

Abstract: Priapism caused by an inflammatory process is rare. We report on a 25-year-old man with priapism due to an infiltration in the pelvic region, enclosing the right sacral plexus. Blood cultures revealed Staphylococcus aureus to be the causal organism. Treatment consisted of parenteral antibiotics, aspiration of the corpora cavernosa and injection of epinephrine, resulting in a flaccid penis and full recovery of potency after 3 weeks. A literature review was conducted for infection and inflammatory processes as a… Show more

“…Inflammatory markers such as circulating C-reactive protein (CRP) and interleukin-6 (IL-6) are predictive of coronary artery disease in the general population [7] and are correlated with poor outcomes in the CKD population [6, 8, 9]. Studies suggest that in maintenance hemodialysis (MHD) patients a paradoxical association exists between low serum cholesterol and increased all-cause and CV death [4,10,11,12]. …”

“…It has been reported in systemic infections such as mumps, Rocky Mountain spotted fever, tularemia, undulant fever, Mycoplasma pneumoniae , Coxsackie B and Echovirus 14 ( 8 , 10 , 21 , 22 , 23 ). Appendicitis, Crohn disease, ulcerative colitis and localized urogenital infections, such as chronic prostatitis and urethritis can cause pelvic inflammation that can lead to irritation the nervi erigentes or pelvic plexus causing a neurogenic LFP ( 9 , 24 , 25 ). There are six reported cases of LFP associated with appendicitis appearing in the literature to date and all of them are males ( 9 ).…”

“…Pelvic inflammation can be caused by appendicitis [5,6,9], Crohn disease [10] and ulcerative colitis [11], and localized urogenital infections such as chronic prostatitis and urethritis and can lead to irritation the nervi erigendi or pelvic plexus causing a neurogenic LFP [12]. This suggests that the etiology of the association between pelvic inflammation and LFP is the abnormal and sustained To our knowledge, this is the fourth reported case of LFP associated with appendicitis appearing in the literature ( Table 1).…”
