Prevention of infection in primary THA and TKA

Abstract: Rates of peri-prosthetic joint infection (PJI) in primary total hip and total knee arthroplasty range between 0.3% and 1.9%, and up to 10% in revision cases. Significant morbidity is associated with this devastating complication, the economic burden on our healthcare system is considerable, and the personal cost to the affected patient is immeasurable. The risk of surgical site infection (SSI) and PJI is related to surgical factors and patient factors such as age, body mass index (BMI), co-morbidities, and lif… Show more

“…These complications are more likely associated with the surgeon’s experience and expertise in performing the operation, as well as the difficulty of the specific procedure itself. Furthermore, the surgeon’s skills are likewise correlated with the time required for performing the surgery—the longer the duration, the higher the risk for SSI [ 19 , 20 , 21 ]. Nevertheless, by systematically analyzing possible reasons for adverse outcomes, case by case, technical aspects can be discussed during an M&M conference in an intercollegial way.…”

“…It is expected that the influence of patient characteristics, comorbidities and medication use is comparable for both THA and TKA. 44 The influence of comorbidities on outcomes is studied together quite often. 3 Furthermore, we tested this assumption by performing the analysis on THA and TKA data only.…”

“…Despite great survival rates and improved outcomes of arthroplasties, various failures and complications were reported, including periprosthetic joint infection (PJI), which is one of the most common failure mechanisms. PJI occurs following primary THA and TKA, with rates ranging between 0.3% and 1.9%, and up to 10% in revision cases [2,3]. PJI is a devastating failure with Polymers 2021, 13, 2240 2 of 14 considerable morbidity and large economic burdens, both for affected patients and the healthcare system.…”
