Prevention of distal embolization and no-reflow in patients with acute myocardial infarction and total occlusion in the infarct-related vessel: A subgroup analysis of the cohort of acute revascularization in myocardial infarction with excimer laser?CARMEL multicenter study

Abstract: To overcome the adverse complications of percutaneous coronary interventions in thrombus laden lesions (i.e., distal embolization, platelet activation, no-reflow phenomenon), mechanical removal of the thrombus or distal embolization protection devices are frequently required. Pulsed-wave ultraviolet excimer laser light at 308 nm can vaporize thrombus, suppress platelet aggregation, and, unlike other thrombectomy devices, ablate the underlying plaque. The following multicenter registry was instituted to evaluat… Show more

“…Moreover, final cTFC was below the prognostic cut off point of 23 [26] in the ELCA group, but not in the control group. The findings in this subgroup are in good agreement with prior reports that also assumed a benefit from ELCA particularly in patients with a high thrombotic burden [11,12]. However, due to the low number of patients, interpretation of angiographic results should be cautious.…”

“…Large-scaled, controlled trials are needed to address the question of superiority and to identify an ultimate device for revascularization therapy in AMI. Although our study could not supply evidence for a routine use of ELCA in STEMI patients, laser angioplasty remains an adjunct to mechanical reperfusion therapy and should still be considered for selected patients with AMI, particularly in the presence of a total thrombotic occlusion [12].…”

“…These actions might exert a beneficial effect on "no reflow", i.e., the failure of microvascular reperfusion due to a combination of platelet microthrombi followed by endothelial swelling and myocardial edema [8]. Results obtained in clinical experiences are encouraging -although somewhat limited by small sample size and limited number of recruiting academic centers [9][10][11][12][13]. At this point the technique seems mature for a larger scale validation.…”
