“…Currently, platelets are known to play an important pathogenetic role in the development of glom erulonephritis by facilitating the deposition of immune complexes and inducing glomerular damage through the release of vasoactive and inflammatory substances such as serotonin and histamine [7], Therefore, treatment with an tiserotonin and antihistamine agents has been considered to protect animals from proteinuria and nephritis. Recently, an inhibitor of thromboxan A2 synthetase has been reported to be effective against the urinary protein excretion asso ciated with bovine serum albumin (BSA) nephritis in rab bits [8] and dipyridamole, alone or in combination with aspirin, has been clinically used as an antiplatelet ther apy [9], Ticlopidine is a selective and strong antiplatelet agent which has been tested under various conditions and has proved efficient in reducing platelet consumption during extracorporeal circulation [10]. Clinically it has been used for diabetic nephropathy, but has been reported to be ineffective in reducing proteinuria [11].…”

The effet of the antiplatelet agents ticlopidine and dipyridamole on immune complex glomerulonephritis in rats was evaluated. Bovine serum albumin (BSA) nephritis was induced in rats with subcutaneous immunization and intravenous dosage of BSA. Ticlopidine and dipyridamole were administered for 4 weeks before and for 7 days after onset of proteinuria. It was shown that both ticlopidine and dipyridamole could reduce the development of proteinuria when administered before the onset of proteinuria. Ticlopidine also reduced the amount of urinary protein after onset of protenuria, and may thus be more effective against urinary protein excretion. When the antiplatelet agents were administered before onset of glomerulonephritis, blood urea nitrogen and serum creatinine levels were significantly lower, and glomerular hypercellularity and mesangial widening were milder in animals treated with ticlopidine than in controls. Glomerular deposition of BSA was less intense in treated animals than in nontreated controls. Thus, ticlopidine as well as dipyridamole was found to inhibit the development of proteinuria and glomerular alteration. It is suggested that ticlopidine could be more effective than dipyridamole against the development of immune complex glomerulonephritis in rats.

Background-Inhibition of platelet aggregation by preventing the binding of fibrinogen to glycoprotein (GP) IIb/IIIa on activated platelets results in antithrombotic activity. We report on the antithrombotic effect of xemilofiban (SC-54684A), an oral GP IIb/IIIa antagonist, administered alone or with aspirin (ASA) in an acute thrombosis model. Methods and Results-Conscious dogs were treated with xemilofiban (1.25, 2.5, 5.0, or 6 mg/kg, nϭ6); low-dose (LD, 81 mg) ASA, nϭ7; high-dose (HD, 162 mg) ASA, nϭ6; xemilofibran 1.25 mg/kg plus LD ASA, nϭ6; xemilofibran 1.25 mg/kg plus HD ASA, nϭ6; or placebo, nϭ7. Dogs were anesthetized 60 minutes later, and the effects of the treatments were evaluated after electrolytic injury (250 A for 180 minutes) in the left circumflex coronary artery. Bleeding time (BT) was assessed in a separate study. Incidence of thrombosis was reduced (PϽ0.05) by xemilofiban Ն2.5 mg/kg, HD ASA, or xemilofiban 1.25 mg/kg plus HD ASA compared with placebo. Xemilofiban Ն2.5 mg/kg or xemilofiban 1.25 mg/kg plus HD ASA significantly increased time to occlusion, inhibited ex vivo platelet aggregation to collagen Ͼ90%, and prevented or decreased (PϽ0.05) cyclic flow variations (CFVs) compared with placebo. BT was increased (PϽ0.05) with xemilofiban Ն2.5 mg/kg but not with xemilofiban 1.25 mg/kg plus HD ASA. Conclusions-Xemilofiban Ն2.5 mg/kg, HD ASA, or xemilofiban 1.25 mg/kg plus HD ASA significantly reduced the incidence of thrombosis. These doses of xemilofiban or xemilofiban 1.25 mg/kg plus HD ASA increased time to occlusion, inhibited ex vivo platelet aggregation by Ͼ90%, and prevented or reduced CFVs. Xemilofiban Ն2.5 mg/kg but not xemilofiban 1.25 mg/kg plus HD ASA significantly increased BT. (Circulation. 1998;98:813-820.)
