Prevention and Management of Pancreatic Fistula

Abstract: Despite significant improvements in the safety and efficacy of pancreatic surgery, post-operative pancreatic fistulae remain an unsolved dilemma. These occur when the transected pancreatic gland, pancreatic-enteric anastomosis, or both, leak rendering the patient at significant risk. They are especially important today since indications for resection (IPMN, carcinoma) continue to increase. This review considers definitions and classifications of pancreatic fistulae, risk factors, preventative approaches and of… Show more

“…This would generate an increased risk of about 10 times compared with hardened or intermediate consistency glands. These results have been reproduced in other studies (5,9,23) . The diameter of the Wirsung duct less than 3 mm was also associated with a 22% rate of fistula, compared to 7% in ducts with a diameter greater than 3 mm (36) .…”

“…In our cases, we used stapler routinely for dividing the pancreas however fistula developed in 2 patients. In case of fistula occurrence, up to 90% of the patients can achieve recovery with conservative approaches as in cases (22). Patients, who are refractory to medical treatment, may require radiologic drainage, and they may rarely need surgery (22).…”

“…In case of fistula occurrence, up to 90% of the patients can achieve recovery with conservative approaches as in cases (22). Patients, who are refractory to medical treatment, may require radiologic drainage, and they may rarely need surgery (22). In some studies, somatostatin analogues were claimed to prevent fistula development (22,23 (15,17,19).…”

“…The reported incidence of pancreatic fistula following pancreatico-duodenectomy is 6-14% [2,3]. However, after distal pancreatectomy, it is much lower (0.5%) [4].…”

“…Those which show resistant to healing will need interventions [1]. Surgical techniques for the treatment of intractable pancreatic fistula include percutaneous transfistulous pancreatic duct drainage (PTPD), percutaneous transfistulous pancreatojejunostomy (PTPJ), and percutaneous transfistulous pancreatic duct embolization (PTPE) [1,2]. This report is based on a non-operative therapeutic strategy, in a young male who developed a cutaneous pancreatic fistula following open drainage of a collection of pus in the pancreatic bed and splenic region.…”
