Preventing Venous Thromboembolism in Medical Patients

Abstract: Abstract-Given the increased number of patients hospitalized for acute medical illnesses and the associated risk of venous thromboembolism (VTE), the use of prophylaxis has become a public health matter. Thromboprophylaxis is not widely practiced in acutely ill medical patients, due in part to the heterogeneity of this group and the perceived difficulty in assessing those who would most benefit from treatment. Nevertheless, the results of recent well-conducted clinical trials support the evidence-based recomme… Show more

“…Although heparin-based anticoagulants have a weight of evidence demonstrating their benefit in preventing VTE in postoperative and medical inpatients (12)(13)(14), they are also associated with higher bleeding complication rates than are antiplatelet medication (15). Traditionally, it was argued that antiplatelet agents were ineffective as VTE prophylaxis because of the prothrombotic milieu in veins featuring platelet-independent, low-flow fibrin aggregation.…”

“…After completion of the audit, we estimated the number needed to prevent one maternal death, assuming that half of episodes are already prevented. 4 This is likely to be an overestimate. This project started as an in-house audit.…”

“…3 Studies suggest that use of low-molecular-weight heparin (LMWH) can reduce VTE in medical patients by 50%. 4 Recent guidance documents on provision of thromboprophylaxis have been published by the Royal College of Obstetricians and Gynaecologists (RCOG) (2009) and National Institute for Health and Clinical Excellence (NICE) (2010). 5,6 RCOG guidance covers all aspects of maternity care.…”
