Problems in the female reproductive system appear at the beginning of the reproductive cycle and persist until hormonal regulation in menopause. The drug therapies currently used can cause intolerance and susceptibility to other diseases, making it important to search for alternative therapeutic approaches that can be adopted during your reproductive life. Women use several species of plants to treat reproductive disorders. Some species are rich in bioactive molecules, such as flavonoids. The present study evaluated the bioactive in silico profile of three flavonoids: tyroside (Kaempferol-3-O-β-D- (6 ”-Ep-coumaryl) glucopyranoside), Pg-1 (5,7-dihydroxy-3,8, 4'-trimethoxy) and vitexin (5,7,4'-trihydroxyflavone-8-C-β-glucopyranoside). The predictive antineoplastic action on organs of the female reproductive system and the protective activity against pathogens (protozoa, bacteria, fungi and viruses) were addressed. The study showed that vitexin had antiviral activity, tiliroside had a better index of antibacterial and antifungal activity and Pg-1 had antiprotozoal activity. The incidence of neoplasms in the female reproductive system is due to hormonal variation and injuries possibly caused by recurrent infections. Therefore, flavonoids, which are considered phytoestrogens, can regulate hormonal disorder and help fight pathogens. Pg-1 had a better breast, uterine and ovarian anticancer potential, while tiliroside had a greater anticancer effect on the cervical region. Vitexin, despite its lower potential compared to other substances, also demonstrated satisfactory breast and cervical cancer activity. Therefore, these molecules can become important candidates for natural drugs with antimicrobial and anticancer activity, contributing to the use of non-synthetic products.

Introduction: Urinary tract infection (UTI) is a common condition in women, especially pregnant women due to the anatamo-physiological changes that occur in their body. Uroculture and Antimicrobial Sensitivity Tests (AST) continue to be the most important diagnostic methods for guiding therapeutic management. Objective: To analyze the prevalence of uropathogens responsible for UTI in pregnant women and their susceptibility profile to the main antimicrobials used in the treatment, in the city of Aracaju-SE. Methodology: This is a retrospective epidemiological study with a cross-sectional design, referring to data linked to urine cultures of pregnant women. The current study analyzed 8066 urine cultures from a microbiological laboratory in the city of Aracaju-SE between May/2018 to May/2020, in order to learn about the etiologic agents of UTI in pregnant women and their respective susceptibility profiles to antimicrobials. Results: As for the increase in microorganisms, Escherichia coli prevailed in 87% of positive urine cultures, followed by other Gram-negative and Gram-positive bacteria. As for susceptibility to antimicrobials, drugs suggested by the FEBRASGO protocol were tested, where it was possible to show a better performance of the so-called first choice antimicrobials such as phosphomycin and nitrofurantoin. Conclusion: Escherichia coli was the uropathogen that prevailed in positive urine cultures in pregnant women. The uropathogens aggregate indices of sensitivity to Fosfomycin and Nitrofurantoin, which makes it safe in its use as first line drugs. As for Amoxicillin/Clavulanate and Cefuroxime, the results are not homogeneous for microbial species, suggesting higher failure rates when compared to the first choice.

Bacterial profile and prevalence of urinary tract infections in pregnant women in Latin America: a systematic review and meta-analysis

Background Given the physiological changes during pregnancy, pregnant women are likely to develop recurrent urinary tract infections (UTIs) and pyelonephritis, which may result in adverse obstetric outcomes, including prematurity and low birth weight preeclampsia. However, data on UTI prevalence and bacterial profile in Latin American pregnant women remain scarce, necessitating the present systematic review to address this issue. Methods To identify eligible observational studies published up to September 2022, keywords were systematically searched in Medline/PubMed, Cochrane Library, Embase, Web of Science, and Bireme/Lilacs electronic databases and Google Scholar. The systematic review with meta-analysis followed the Preferred Reporting Items for Systematic Reviews and Meta-Analyses guidelines, and the quality of studies was classified according to the Strengthening the Reporting of Observational Studies in Epidemiology guidelines. The meta-analysis employed a random-effects method with double-arcsine transformation in the R software. Results Database and manual searches identified 253,550 citations published until September 2022. Among the identified citations, 67 met the inclusion criteria and were included in the systematic review, corresponding to a sample of 111,249 pregnant women from nine Latin American countries. Among Latin American pregnant women, the prevalence rates of asymptomatic bacteriuria, lower UTI, and pyelonephritis were estimated at 18.45% (95% confidence interval [CI]: 15.45–21.53), 7.54% (95% CI: 4.76–10.87), and 2.34% (95% CI: 0.68–4.85), respectively. Some regional differences were also detected. Among the included studies, Escherichia coli (70%) was identified as the most frequently isolated bacterial species, followed by Klebsiella sp. (6.8%). Conclusion Pregnant women in Latin America exhibit a higher prevalence of bacteriuria, UTI, and pyelonephritis than pregnant women globally. This scenario reinforces the importance of universal screening with urine culture during early prenatal care to ensure improved outcomes. Future investigations should assess the microbial susceptibility profiles of uropathogens isolated from pregnant women in Latin America.
