This study aimed to analyze the spatial distribution of chikungunya cases in Piauí between 2015 and 2019. This was a spatial ecological study of notified and confirmed cases of chikungunya, with data from the Notifiable Diseases Information System (SINAN) from 2015 to 2019. The X² association test was applied for the bivariate analysis and the spatial analysis was performed from the data treatment to combine with the cartographic base in the free software Qgis (version 3.16.7 Hannover). During this period, 9596 cases and 7950 confirmed cases of chikungunya were reported. The city of Teresina (53.21%) and the health region Entre Rios (58.27%) had the highest records. Females (58.40%), aged between 20 and 34 years (29.63%) and mixed race (50.26%) were the most affected. Regarding education, 56.55% were ignored and, regarding the months of notification, the months of the 1st semester had the highest number of cases (62.8%). Chikungunya cases are concentrated in poles with large population flows. In this way, the identification of the epidemiological profile, as well as its main risk factors, is a way of helping the health system of the entire state in the elaboration of specific control policies for the population most vulnerable to the disease.

Chikungunya fever is a viral disease transmitted by the Aedes aegypti or Aedes albopictus mosquito infected with the Chikungunya virus. Symptoms include sudden fever, severe joint pain, arthritis, tenosynovitis, skin rash, myalgia, in addition to severe neurological and cardiac manifestations. The disease is endemic in all Brazilian states since 2014, with high incidence and morbidity. In 2017, it is worth highlighting the Chikungunya fever epidemic that occurred in Brazil, in which cases increased dramatically, causing serious damage to public health. Thus, the present work aims to list and compare the indicators of incidence of Chikungunya fever in the years 2017 and 2021 in the state of Tocantins and in Brazil (in general), in addition to discussing possible justifications for the data found. Given this context, an ecological study was carried out, of an epidemiological and comparative nature, through the collection of data from the Notifiable Diseases Information System (SINAN), followed by the analysis and interpretation of the data obtained. The study analyzed the incidence in Tocantins in the years 2017 and 2021, recording 6,615 and 1,140 cases, respectively, with an incidence of 0.48% and 0.08%. In Brazil, 245,905 cases were registered in 2017 (incidence of 0.11%) and 136,208 in 2021 (incidence of 0.06%). There was a significant drop in incidence indicators in both populations between 2017 and 2021, and the disease had a higher incidence in Tocantins than in the general Brazilian spectrum. In Tocantins, there was a higher incidence of cases in females and the age group with the highest incidence was the elderly. Therefore, in view of the scenario analyzed, it is inferred that Chikungunya fever may have the tropical climate and/or the occurrence of migrations as an expansion factor, in addition to having women and the elderly as the groups most susceptible to infection.

A Chikungunya é uma doença artrítica debilitante causada pelo vírus Chikungunya. Essa doença é considerada, pela saúde pública, como de preocupação global e crescente, cuja agente etiológico é um alfavírus, transmitido pela picada de fêmeas de mosquitos Aedes aegypti ou Aedes albopictus. Chikungunya é de origem africana, de onde tem se espalhado pelo mundo, causando alto número de epidemias na Ásia, Europa, Américas e Ilhas do Pacífico. O início dos surtos de Chikungunya são da década de 1960, seguindo-se de diminuição para casos esporádicos e ressurgimento em 2006. No Brasil, especificamente, o primeiro registro da doença foi descrito em 2010, no Rio de Janeiro -RJ, como um caso importado da Indonésia. Contudo, a partir de 2014, surgiram relatos de infecções autóctones causadas por CHIKV no Amapá e na Bahia com o consequente aumento do número de casos. Em virtude dessa crescente taxa de infecção, da alta infestação pelo vetor e as consequentes chances de grande disseminação da doença no Brasil, faz-se necessário abordar a importância epidemiológica da Chikungunya em regiões endêmicas e discutir as manifestações clínicas, diagnóstico, tratamento e medidas profiláticas.
