Prevalence of Tuberculosis in a Prison in Tehran by Active Case Finding

Abstract: The low TB prevalence within prisoners besides considering likely common symptoms in ACF may result in many false positive cases during screening (6.5% vs. 0.13%). But, ACF is more fast, easy and affordable method to find TB suspected prisoners. Also, during this active method, prisoners will be trained about TB symptoms so, it has more permanent effect towards screening goals.

“…Five papers related specifically to the prevalence of TB in prisons or in those with a history of incarceration; each paper related to a different country (India (26), China (27), Ethiopia (28), Iran (29) and Tunisia(30)). Tong et al (2019) (27) used cluster sampling methods to gather evidence of TB prevalence in two randomly chosen prisons in middle China.…”

“…Compared with the province's and China's general population, prisoners had 1.65 (95% CI: 1.13,2.41) and 2.26 times (95% CI: 1.65,3.11) increased odds of having active TB respectively. Farhoudi et al (29) used active case finding to screen for TB-suspected prisoners (self-report in response to health care staff explanation of symptoms) in Great Tehran Prison who were then referred to a clinic for sputum testing: this method identified a 'low' prevalence of 6.5% (448/6,900) TB-suspected prisoners and only nine were subsequently diagnosed (0.13%).…”

“…Active health service provision (AHSP), a new model for Iranian health service provision, has previously been described in detail in national clinical guidelines [8] and aims to restructure the health service delivery system to actively and effectively find, treat and follow TB patients. The JHR 34,6 aim of this study was to compare the defined indicators of TB in the intervention and control prisons after implementation of AHSP for TB and HIV management in Iranian prisons.…”
