Introduction: Hepatitis C virus (HCV) infection is an emerging public health issue affecting young people in developing countries. It is the common cause of chronic liver disease which if untreated, leads to cirrhosis, hepatocellular carcinoma and ultimately liver transplantation. HCV infection is conventionally treated with Interferon-α (IFN-α) and anti-viral Ribavirin (RBV). Thyroid dysfunction is frequently encountered in patients having HCV and being treated with IFN-α and antiviral (RBV).Objective: To find out the incidence of thyroid dysfunction i.e., hypothyroidism in patients of HCV infection being treated with IFN-α and anti-viral (RBV). Material and Methods:250 patients having HCV infection were enrolled for the study. All patients were treated with 3 million units subcontinually 3 times per week and antiviral (RBV). Thyroid function tests were performed at the start of the treatment to have a baseline thyroid status and at the end of the treatment. Patients were considered to have hypothyroidism when thyroid stimulating hormone (TSH) was more than 4.0 mIU/L.Results: 47 (18.8%) patients had thyroid dysfunction after 3 months of the therapy. Out of these 47 patients, 36 (76.59%) had hypothyroidism and 11 (23.4%) had hyperthyroidism. It is evident that patients on IFN-α and antiviral therapy for HCV infection developed thyroid dysfunction particularly hypothyroidism that is more in females.

The objective of this study was to investigate whether IRF7 polymorphisms are associated with autoimmune thyroid diseases (AITDs). We selected three single nucleotide polymorphisms (SNPs) of IRF7, namely, rs1061501, rs1131665, and rs1061502 for genotyping using PCR-based ligase detection reaction (LDR) method in a total of 1659 participants (592 with Graves' disease, 297 with Hashimoto's thyroiditis, and 770 healthy controls). Gene-disease and genotype-clinical phenotype associations were evaluated for the three SNPs. Our results showed that the AG genotype and the minor allele G frequency of rs1131665 and rs1061502 in AITD patients were both higher than those of the controls (rs1131665: AG genotype: P = 0.017, OR = 1.968; allele G: P = 0.018, OR = 1.946; rs1061502: AG genotype: P = 0.029, OR = 1.866; allele G: P = 0.031, OR = 1.847). Subgroup analysis also showed that the AG genotype and the minor allele G frequency of rs1131665 and rs1061502 in Graves' disease patients were both higher than those of the controls (rs1131665: AG genotype: P = 0.015, OR = 2.074; allele G: P = 0.016, OR = 2.048; rs1061502: AG genotype: P = 0.034, OR = 1.919; allele G: P = 0.035, OR = 1.898). Furthermore, the allele G frequency of rs1061501 was associated with Graves' ophthalmopathy (P = 0.035, OR = 1.396). No significant difference in IRF7 polymorphisms was found between Hashimoto's thyroiditis patients and controls. Our study has revealed for the first time that IRF7 is a susceptibility gene for AITD, especially for Graves' disease and Graves' ophthalmopathy.

The recent development of direct‐acting antiviral (DAA) drugs has revolutionized the area of hepatitis C virus (HCV) therapeutics but the efficacy and clinical outcome of interferon (IFN)‐free therapy have not been extensively studied yet. We observed a dramatic increase in hypothyroidism among patients treated with sofosbuvir, IFN, and ribavirin. This is the first prospective study of the thyroid dysfunction in DAA drugs treated patients. This study compared the risk of hypothyroidism in two different groups of HCV patients treated with different DAA drugs regimens that were sofosbuvir + pegylated‐IFN‐α + ribavirin and sofosbuvir + daclatasvir + ribavirin. Our findings highlight the periodic screening of serum thyroid‐stimulating hormone and T4 levels in HCV infected patients during the treatment and posttreatment.
